1.24 The Site is located inside the UP Cebu Campus, Gorordo Avenue,
Lahug, Cebu City.

1.28 The Start Date shall be the date of receipt of the Notice to Proceed.
1.31 The Works consist of:

Part I. Construction Phase:


1) General Requirements:
I. Description of the Project
 Furnish all materials, equipment, plant tools, and perform
labor to complete all works in the Rehabilitation of
Comfort Rooms in UP Cebu (Administration, High School
& A.S. Building), in accordance with the plans,
specifications, the Philippine Bidding Documents, the
Terms of Reference and other related contract
documents.

II. Permits, Licenses and Taxes


 Secure and pay all permits, fees, licenses, taxes, etc.
Necessary for the execution of the general construction
works.
 Prepare a monthly progress report which shall include,
among others. An overall progress chart and progress
value of construction work.
 Provide project Billboard in accordance with the COA
circular 2013-004 dated Jan. 30, 2013 under Annex “A
and other signage as required

III. Demobilization and Cleaning


 Demobilize, dismantle and remove all temporary
facilities, including all workmen’s house, construction
equipment, tools, personnel and debris out of the project
site and / or University premises
 Clean building and site; spic and span, ready for use.
Temporary Facilities and Utilities;

3) Architectural Works/ Carpentry works;


4) Sanitary/Plumbing Works;
5) Electrical Works;

5.1 The Procuring Entity shall give possession of all parts of the Site to the
Contractor at the Start Date.
6.5 The Contractor shall employ/commission the following Key Personnel:
For Construction Phase: Project Manager/ Architect / Engineer and
General Foreman

NOTE: The names of the Key Personnel and their designation shall be
filled out by winning contractor prior to contract signing.

1. Location of the existing tapping points of sanitary/plumbing,
electrical.

classified under the National Building Code of the Philippines and other
structures made of steel, iron, or concrete which comply with relevant
structural codes (e.g., DPWH Standard Specifications), such as, but not
limited to, steel/concrete bridges, flyovers, aircraft movement areas,
ports, dams, tunnels, filtration and treatment plants, sewerage systems,
power plants, transmission and communication towers, railway system,
and other similar permanent structures: Fifteen (15) years.

29.1 No dayworks are applicable to the contract.


31.1 The Contractor shall submit the Program of Work to the Procuring
Entity’s Representative within Thirty (30) days of delivery of the Letter
of Acceptance.
31.3 The period between Program of Work updates is Thirty (30) days.
The amount to be withheld for late submission of an updated Program
of Work is 1/50 of 1% of contract value.
34.3 The Funding Source is the Government of the Philippines.
39.1 The amount of the advance payment is Fifteen percent (15%) of the
Contract Value and can be availed of upon the submission and receipt
of a request for the release of the advance payment after the receipt of
the Notice to Proceed (NTP) and posting of an irrevocable standby letter
of credit of equivalent value from a commercial bank, a bank guarantee
or a surety bond callable upon demand, issued by a surety or insurance
company duly licensed by the Insurance Commission and confirmed by
the Procuring Entity.
40.1 Materials and equipment delivered on the site but not completely put
in place shall not be included for payment.
51.1 The date by which operating and maintenance manuals are required is
after the issuance of final completion and acceptance.

The date by which “as built” drawings are required is fifteen (15)
calendar days after issuance of final acceptance.

51.2 The amount to be withheld for failing to produce “as built” drawings
and/or operating and maintenance manuals by the date required is
1/50 of 1% of contract value.
