I hereby authorize my mother, SUSANA R. MIGUEL, to claim the allowance that

will be received from BRO for Education Scholarship since I am currently having my On-
the- Job Training (OJT) in Manila, Philippines. Thus, I will not be able to attend the
General Assembly and the Release of Allowance that will be held.
