Inspection and Test Plan

In process and final inspection and testing shall be undertaken for procurement,
construction, installation and commissioning activities. The extent of the inspection
and testing shall be determined by the requirements of the standards and
specifications, and the safety criticality of the material, equipment, construction and
operating system.
Contractors and Suppliers shall identify their inspection and test activities
in Inspection and Test Plans (ITPs) that shall include:

a. Designation of the item, sub-assembly, material type etc.


b. The work sequence.
c. The responsible discipline/party on each sequence
d. The standards to be applied and the qualification of the process
e. The acceptance criteria
f. Verifying Documentation/Records
g. Type of Inspection (Hold, Witness, Monitoring, Review)
h. The inspection authorities, both internally and externally

DEFINITIONS :

MONITOR (S or M)
Work in process proceeds but an Inspector may invoke inspection by attributes as
necessary. Also known as “Patrol Inspection”.

WITNESS (W)
Work in process may not proceed beyond this stage without inspection or an
authorized waiver from the client. Usually applied to a mandatory production check
where third party verification may be required. A 24 hour notification period will be
given and if no attendance by the Company/Certifying Authority within that period,
work will continue.

HOLD POINT (H)


The stage in the manufacturing process where inspection or test must be undertaken
before work may proceed. Applied to various fabrication inspection activities e.g.
welders qualification, pressure test, final test, acceptance tests, mandatory safety
checks, etc. A 24 hour notification period will be given and if no attendance by the
Company/Certifying Authority within that period, work will continue.

REVIEW (R)
Review instruction/documentation/comment. If required but do not hold up work.

RESPONSIBLE PARTY
A discipline/person responsible for the particular steps /process in the ITP.

Normally, ITP is one document in addition to Project Quality Plan. ITP’s shall be
approved by the Client prior to execution of the particular activities

The Project Team shall identify their inspection requirements in the Contractors,
subcontractors and vendors Inspection and Test Plans. In addition to the inspections
noted in ITPs, the projects Site Teams have the flexibility to instigate surveillance
programs independent of agreed ITPs. The levels of inspection and surveillance will
be based upon the criticality of an item and the performance of the Contractor in
achieving the required quality. The involvement of any Regulatory Authority or Third
Party Inspection shall also be included in the ITP.

Suitably competent and qualified personnel shall undertake the inspection & testing
activities. All Contractor inspectors will be assessed for capability.
Inspection, measuring and monitoring equipment shall be controlled, calibrated and
preserved in a satisfactory condition.
Non-conformities detected during inspection or testing shall be noted and appropriate
corrective action taken to rectify the non-conformity.
Records shall be issued for all surveillance, inspection and testing, and the inspection
and test status identified.
