AFFIDAVIT OF BEING SINGLE

I, JONEL B. LANUSGA, Filipino, of legal age, single, and a resident
of Sitio Baja, Brgy. Hibago, Ocampo, Camarines Sur, under oath states
that:

1. I am applying in the Military Service as a trainee in the Philippine

Army;

2. That as one of my requirements, I am declaring, for all intents and

purposes, that I am still single and is not yet and have never been
married;

3. Similarly, I hereby declare that I have no child and have not yet
fathered a child;

4. I am executing this Affidavit to attest to the truth of the foregoing and

for whatever lawful purpose that it may serve.
