AFFIDAVIT
I, AIDA MANZANO ENCOY, of legal age, married, Filipino and a resident of
Zone 3, Liberty, Laguindingan, Misamis Oriental, after being sworn to in accordance
with law, do hereby depose and say:

1. That I am married to Ceasar A. Encoy having celebrated our marriage on

September 26, 1994;

2. That my husband Caesar A. Encoy is an Overseas Filipino Worker currently

working at Saudi Arabia as a driver in a particular company;

3. That early in the morning of November 9, 2017, my husband messaged me in my

messenger account informing me that he has sent money to me in the amount of
Php 19,613.00. with tracking number QPAY611173122688;

4. That in the afternoon of the same date, I went to Cebuana Llhullier to claim the
money. To my surprise, there was a discrepancy with regard to my name which
is written as “Aida E. Manzano” instead of "Aida M. Encoy”;

5. That I make this statement for the purpose of convincing Cebuana Lhuillier to
release the amount to my favor;

6. That I executed this affidavit in good faith to affirm the foregoing facts for purpose
stated above;
