AFFIDAVIT OF DESISTANCE

I, ___________, of legal age, Filipino and a resident of

_______________, Quezon City, after having been duly sworn to in
accordance with law, do hereby depose and state, THAT:

1. I am the complainant in Criminal Case No. __________

for ________ now pending before the Metropolitan Trial
Court, Branch _______, Quezon City;

2. After introspective contemplation of the facts, I realized

that the filing of this case was the result of
misunderstanding and misapprehension of facts;

3. Hence, I am no longer interested in prosecuting this case

against the accused ______________;

4. I am filing this Affidavit of Desistance for the purpose of

moving for the DISMISSAL of the aforesaid criminal
charge against the accused _________________;

5. In executing this Affidavit, I am not coerced, bribed, or

intimidated, and I am fully aware, that if the instant case
will be dismissed by this Honorable Court it can no longer
be revived.
