AFFIDAVIT OFCONSOLIDATION

I, APOLLO Q. CABUTAJE, Filipino, of legal age, married to Vilma

Hernandez-Cabutaje and resident of Centro, Lasam, Cagayan after first having
been duly sworn to in accordance with law depose and state:

1. I am the registered owner of two (2) parcels of land covered by TCT Nos.
T-162370 and T-119512;

2. That the above properties are adjacent to each other and situated in
Centro II, Lasam, Cagayan;

3. That as a new owner, I am of the intention to consolidate ownership over

the two (2) properties in order that I will be issued a single title;

4. That I have executed this affidavit to attests to the truthfulness of the

foregoing statements and to facilitate the consolidation of both properties
to give way to the issuance of a single title.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 11th

day April 2011 in Tuguegarao City, Cagayan, Philippines.
