To whom it may concern;

Good day!

I would like to request your good office to refund the enrolment fee I have paid for the
1st term of the academic year 2018-2019 on the subject, “Independent Study for Educational
Innovation”, with the subject code ED704, under DR. ENRICO GARCIA, in room 207 of the Main
Building, scheduled every Saturday from 3:30-6:30pm.

I have already taken this subject on my 3rd term in the academic year 2017-2018, but
because of some mistakes in encoding, this subject was entered again on my 1st term in the
academic year 2018-2019 instead of another. This request was approved already by the
important signatories. However, due to my failure to submit the Request Form to the
accounting office, because of my lack of knowledge of the procedure, its execution was
delayed. I submit this letter to the accounting office hoping that your good office would allow
the continuation of the process.

I am hoping for your positive response with regards to this matter!
