AFFIDAVIT

I, DANILO ANDRADA y Campos, of legal age, married and a

resident of Barangay Garitan, Tagudin, Ilocos Sur, after having been sworn to
in accordance with law do hereby depose and state the following to wit:

1. That my late brother Alfredo Campos Andrada is the registered owner

of one (1) Honda TMX155RT motorcycle without side car bearing plate
no. 5080ZL;

2. That after he died, I took custody of the above-described motorcycle;

3. That I attached a sidecar on the said motorcycle for private and family
use;

4. That I am executing this Affidavit for the purpose of changing the

registered denomination of the said motorcycle to the Land
Transportation Office and for all legal intents and purposes it may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this
