Acknowledgement, Release, and Agreement to Participate

In consideration of the United States Tournament of Dance, Inc. (“USTD”), Millennial Event Group (“Millennial”),
Julian Deguzman, Joshua Price, Bella Maisonett, permitting me , to take part in
USTD and Millennial’s “The New Wave” (TNW), I acknowledge and agree that:

1. By signing and submitting this form I signify that I have read, understand, and agree to all abide by the
rules and regulations of “TNW” and all other aspects in connection with it.

2. I represent that I am an amateur dancer between the ages of seven (7) and forty (40) and in good health.
If I am a minor, my parent or legal guardian has granted his/her permission for me to participate in
“TNW” as evidenced below.

3. I am fully aware that there are dangers and risks (direct and indirect) inherent to “TNW,” including but
not limited to the possibility of physical injury (including death) to myself and/or others resulting from
“TNW.” I expressly assume all such dangers and risks relating to or arising out of participation in “TNW,”
whether or not arising out of the negligence of “USTD,” “Millennial,” “TNW”, Julian Deguzman, Joshua
Price, Bella Maisonett, and/or any other Releasee (as defined below) or otherwise. I understand that I
must provide my own medical insurance. I hereby authorize “USTD”, “Millennial”, and “TNW” Convention
representatives to act for me according to their best judgment in an emergency requiring medical
attention.

4. I hereby irrevocably grant “USTD”, “Millennial”, and “TNW” the right to photograph, edit, use and re-use,
(collectively, “Use”) and authorize others to “Use” worldwide in perpetuity, in any and all media, whether
now known or hereinafter created, my name, likeness, and biographical information, including pictures,
poses, performances, statements, and sound materials of myself at any point during “TNW” and/or any
related activity in connection therewith on film, videotape, audio tape, and/or otherwise telecast
recorded by or on behalf of “USTD”, “Millennial”, and “TNW” (“the Materials”).

5. I further irrevocably grant the use of my name, likeness, image, appearance, performance, address (town
and state) by “USTD”, “Millennial”, “TNW”, and it’s designees as part of “the Materials” or otherwise for
advertising and publicity purposes relating to “TNW”, “The Materials,” “USTD”, and “Millennial” without
additional compensation, worldwide in any and all formats and media in perpetuity.

6. I hereby forever release and discharge “USTD,” “Millennial”, Julian Deguzman, Joshua Price, Bella
Maisonett, and each of their respective owners, parent companies, (whether direct or indirect) affiliated
entities, advertising and promotion agencies, governing leagues, partners, directors, officers, agents,
employees, licensees, successors, and assigns of any of the foregoing (collectively with, “USTD”,
“Millennial”, “TNW”, “the Releasees”) from and against any and all causes of action, claims, suits,
controversies, agreements, promises, judgments, demands, or claims whatsoever, that I, or my heirs,
executors, administrators, successors, or assigns have or hereafter, at any time, shall or may have against
“the Releasees,” arising out of or in connection with participation in “TNW” whether or not arising out of
negligence of “USTD,” “Millennial”, “TNW”, Julian Deguzman, Joshua Price, Bella Maisonett and/or any
other Releasee otherwise.
