UNIFORM / LAB COAT
CGH
To establish procedures for issuing, maintaining, replacing, and wearing of uniforms and related
uniform accessories.
SECTION 1 – APPLICABILITY

This policy applies to all employees’ uniformed operations.

SECTION 2 – POLICY

1. Uniforms/lab coats will be purchased and provided to career employees by C G H.


2. Each new employee will receive Two uniforms / lab coat annually (Complete Two Set)
3. All personnel (career and volunteer) in violation of this policy will be subject to disciplinary action
consistent with C G H Policy.
4. All department uniforms / lab coats not purchased by the individual.
5. Personnel must not wear or use any uniform / lab coat, or accessory except those approved by this
policy.
6. The HOD must approve any alterations to the uniforms. The exception to this is for fitting
alterations.
7. Personnel who are on official business, including training, representing the department, and who
have been provided advanced notice of the detail, must wear an appropriate uniform / lab coat.

SECTION 3 – ACCOUNTABILITY

1. All personnel are required to produce, upon request by the HOD, any issued uniforms / lab coat or
accessories. An inventory of all issued uniforms / lab coat and accessories is to be on file with
Store Department.
2. All personnel must report the loss or damage of any uniform / lab coat items to their supervisor /
HOD. The lost or damaged item will be replaced at the department's expense, if such loss is found
to be either service connected or not resulting from negligence. If the loss or damage is found to
result from gross negligence, the item will be replaced at the employee's expense. The HOD will
make the final determination as to who is responsible for replacement.
SECTION 4 – UNIFORM ALLOTMENTS

1. Uniforms / lab coats will be issued in accordance with the C G H policy. Depend upon
assignment, variations approved by the HOD.
2. The uniforms / lab coats will be re issued to the permanent employees after completion of
every fiscal year. There is a need for indenting or giving demand to the store department for the
issuance of uniforms after the approval of HODs.
SECTION 5 – RESPONSIBILITIES

1. It is the responsibility of all personnel to be neat and presentable at all times. The favorable public
image of the department depends, in part, on the appearance of each individual member. This
includes keeping all shirts tucked in at all times, shoes clean, and in serviceable order.
2. Personnel who have been issued uniforms / lab coats and accessories are responsible for the care

and maintenance of each item.

SECTION 6 – CLASSES OF UNIFORMS

1. Security Staff: The Security Staff uniform is the full dress uniform. The uniform shall be
worn at formal department activities. The Security Staff uniform consists of the following:

a) Male Staff:
i. Dark Green Color long sleeved button down shirt and Pant (Supervisor and Staff ).
ii. Black leather, metal open latch in good working order, not faded or worn. Belt must not
exceed 2” in width.
iii. The dress cap is to be worn at all duty times while outdoors.
iv. Footwear must have a plain, rounded safety-toe with the heel of the shoe not exceeding 1
inch in height.
b) Female Staff:
i. Dark Green Color long sleeved Kameez and Shalwar with Green Dupatta .
ii. Footwear must have a plain, rounded safety-toe with the heel of the shoe not exceeding 1
inch in height.

2. Transport Staff: The Transport Staff uniform is the full dress uniform. The uniform shall be
worn at formal department activities. The Transport Staff uniform consists of the
following:

a. Sky Blue Color long sleeved button down Shirt and Dark Blue dress Pant (In-charge / Staff
Members).
b. Black leather, metal open latch in good working order, not faded or worn. Belt must not exceed
1” in width.
c. Footwear must have a plain, rounded safety-toe with the heel of the shoe not exceeding 1 inch in
height.

3. Repair & Maintenance Staff: The Repair & Maintenance Staff uniform is the full dress
uniform. The uniform shall be worn at formal department activities. The Repair &
Maintenance Staff uniform consists of the following:

a) Dark Blue Color long sleeved button down Shirt and Dark Blue Trouser (Staff Members).
b) Safety Shoes available at particular shops in market as PPE’s for the Electrical/Mechanical
& Civil Workshops.
4. Front Desk Officers: The front desk officers uniform is the full dress uniform. The uniform
shall be worn at formal departmental activities. The front desk officers uniform consist of the
following:

a) Female staff:
Light Blue Color Shirt, Dark Blue Color Shalwar and Dupatta and Black Shoes with

White Socks.
b) Male staff:
Light Blue Color Shirt, Dark Blue Color Dress Pant and Black Shoes with
White Socks.

5. Out Patients Department (OPD) Staff: The OPD staff uniform is the full dress uniform.
The uniform shall be worn at formal departmental activities. The OPD Staff uniform consist
of the following:

a) Female Staff:
Pink Color Shirt and Shalwar, Maroon Color Dupatta and Black Shoes with White Socks.
b) Male Staff:
White Color Shirt, Black Color Dress Pant, Black Shoes with White Socks.

6. Dental, Physiotherapy and Ultrasound Department Staff: The Dental, Physiotherapy and
Ultrasound Department Staff uniform is the White Lab Coat. The uniform shall be worn at formal
departmental activities.

7. Diagnostic Lab, Pharmacy & Quality staff: The entire Lab, Pharmacy & Quality staff uniform is
the White Lab Coat. The uniform shall be worn at formal departmental activities.

8. Nursing Staff: The Nursing Staff uniform is the full dress uniform. The uniform shall be
worn at formal department activities. The Nursing Staff uniform consists of the following:
a) Female staff:
White Color Shirt, Shalwar, Jacket with long sleeved, Dupatta and Black Shoes with White
Socks.
b) Male staff:
White Color Shirt, Pant, Black shoes and socks.
c) Unit Receptionist and Coordinator:
Sand Castle Color Shirt, Shalwar and Maroon Color Dupatta, Black Shoes with White
socks.
d) Aya:
Light Blue Color Shirt, Shalwar and Dupatta, Black Shoes with White Socks

9. HIMS Staff: The HIMs Staff uniform is the full dress uniform. The uniform shall be worn at
formal department activities. The HIMs Staff uniform consists of the following:
a) Female staff:
Light Blue Color Shirt, Dark Blue Color Shalwar and Dupatta and Black Shoes with
White socks.
b) Male Staff:
Light blue Color Shirt, Dark Blue Pant, Black Shoes and socks.

SECTION 8 – OFF DUTY WEAR OF UNIFORMS

1. Career personnel are not allowed to wear uniform while off duty except while on the way to and
from their designated work place or while on special assignment with prior approval from HOD.
Personnel, who will be conducting personal business, either before or after their shift.
2. Personnel who are suspended from all activities of the department shall not wear any part of the
uniform or display any department insignia.
3. Uniform items will be replaced at the individual's expense if they are lost or damaged while an
individual is off duty.

SECTION 9 – RESIGNATION/TERMINATION OF EMPLOYMENT

1. In the event of resignation / termination from the department, all uniform clothing and uniform
accessories not purchased by member must be returned to the Store Department in clean condition.
In case of not retuned, deduction to be made against uniforms / lab coats from the employees.
2. Deduction to be made as following against Uniforms / Lab Coats, from employees who are leaving
the Organization:
a) Concrete charges of Uniform/Coat will be deducted if employee leaves the Organization within
2 months.
b) 50 % of concrete charges will be deducted within 6 months.
c) No Deduction will be made after 1 year completion.

3. Under no condition will an employee who is leaving the department exchange items issued by the
department with other members.
