2018-2019 Music Participation Patches/Pin Order Form

In order to receive the patches/pins noted below, you must complete this form. There is a $10 fee (per year,
per student) to help defray the cost of these items. Enclose this form w/check or cash in an envelope marked
"Patches/Pins Order" and drop it in the Band or Choir Mailbox no later than April 1, 2019.
