2. Please be informed that on or about 6:00 AM of March 02, 2019, PNP

personnel from Bansalan Municipal Police Station led by PO2 Edwin V Yeso, Warrant
PNCO and PO2 Bryan D. Magallanes, Assistant Warrant PNCO together with the
personnel from 1104th MC RMFB11 led by SPO2 Carmar P Lee, Intel operative and
PO1 Nicol John Lucero, intel operative and PIB DSPPO led by SPO1 Neil B. Jugadora
successfully arrested one Jerome Lonzaga, male, 24 years old, single and a resident of
Brgy Union, Bansalan, Davao del Sur.

3. Said person was arrested at Rose St. Poblacion Dos, Bansalan, Davao
del Sur by virtue of Warrant of Arrest issued by Hon. Loida Posadas-Kahulugan of RTC
Branch 21 Bansalan, Davao del Sur with Criminal Case Nr. XXI-1852 (14) for
Carnapping with recommended bail bond of One hundred eighty thousand Pesos
(180,000.00).

4. Said arrested person was apprised of his rights under the constitution and
had undergone booking and documentation for proper disposition of the case. IOC PO2

5. Request acknowledgement receipt.
