2. On March 5, 2024 at around 10:00 AM, PNP personnel from Bansalan

Municipal Police Station (lead unit) led by PSsg Edwin V Yeso (Warrant
PNCO), PCpl Patrick P. Cabaca (Intel Operative) under the supervision of
PCAPT SHIELA MAY SWAIN PANSOY, OIC together with the joint
operating units from RIU 12 (co lead) by PCPT ERROL ANGELO
NAULGAN,39th IB under the supervision of LTC ANTHONY FER S
SOLAMO INF(GSC)PA, CIDG PFU Davao del Sur personnel under the
supervision of PMAJ DEANE BENJIE V. SANTOS, 1st DSPMFC personnel
under the supervision of PMAJ RICHEEN M LAGNAYO, OIC, 2nd Company
DSPMFC Personnel under the supervision of PLTCOL DARWIN E TAYPIN
Force Commander 2nd DSPMFC, , PIU-DSPPO personnel under the
supervision PMAJ PETER GLENN D IPONG CHIEF, PIU, 1104th RMFB
personnel under the supervision of PMAJ JOHN GLENN D SABLAN,
COMPANY CMDR, successfully served the warrant of arrest of one Gracelyn
Gore Tacadao alias YADZ (FSMR/ Regional Medic member) 35 years old,
single, and a resident of Purok 1 ,Brgy Nabundasan,Tulunan, North Cotabato.

3. Subject warrant was served at Purok 11., Brgy. Pob.Makilala, North

Cotabato issued by HON. CARFELITA P CADIENTE-FLORES Acting Judge
RTC 11, Branch 21 , Bansalan, Davao del Sur on October 19, 2020 with CC
No.XXI-2501(20) for Attempted murder with recommended bailbond
amounting to one hundred twenty thousand pesos (Php 120,000.00 ).

4. Further, aforesaid wanted person was apprised of his rights under the
constitution and had undergone booking and documentation and is now under
the custody of 39th IB in Makilala, North Cotabato for proper disposition.

5. Attached is the mug shot of the arrested person and her WOA.
