4. WHO :

A. Accused: With initial name MAV (CICL), 16 years old

(DOB: January 23, 2006), minor, (POB: Tanauan, Leyte), Out of School Youth and
resident of Brgy. Sto. Niño, Tanauan, Leyte).
B. Operating Unit: Tanauan MPS led by PCPT KEMY G
BAS, OIC together with PEMS Ruperto E Quiero, PSSg Adrian D Miranda, PSSg
Rodrigo A Redoña Jr., PSSg Ryan M Creer, PSSg Heide G Costimiano (WCPD).

5. WHY : For having standing Warrant of Arrest for the Crime of THEFT
(RPC Art. 308) docketed under CC No. CB-23-1028 issued by CONSTANTINO
FALLER ESBER, Executive Judge of RTC 8, Branch 37 Caibiran, Biliran issued on
May 23, 2023 with recommended bail bond in the amount of Php 10,000.00 pesos.

HOW : That on the said date, time and Place of Occurrence, said accused
accompanied by his mother Reyna Caliwan Abarca went to this MPS for National
Police Clearance (NPS) and that upon verification of PSSg Miranda and PSSg
Redoña Jr. it was found out that said subject person having existing Warrant of
Arrest for said crime. Consequently, WOA was verified and printed thru E-Warrant
System and was subsequently read and served to the aforesaid minor accused in
the presence of his mother and WCPD PNCO. Subject person was informed by the
nature of his arrest, apprised of his constitutional rights as provided by law in the
dialect which is spoken and understood by him and likewise informed that the video
coverage was taken during the said operation in compliance with A.M. 21-06-08-SC
(Rules on the Use of Body-Worn Camera in the Execution of Warrant) through the
use of Body-Worn Camera and Alternative Recording Device.

6. Aforesaid accused is now under the custody of this MPS for

documentation and for proper disposition prior turned over to the court of Origin. IOC
