1. Reference: Memorandum from Chief, Homicide dated May 15, 2023.

2. Above reference pertains to the scheduled meeting of shift supervisors at 4:00

PM, this date wherein the undersigned failed to attend.

3. Please be informed that on May 15, 2023, the undersigned attended the
Closing Ceremony of the Sportsfest held at National Capital Region Training Center.
Further, a direct message was sent by the undersigned to PCPT DENNIS N TURLA,
Chief, Homicide stating therein that the undersigned was presently at NCRTC attending
the above-cited event.

4. The undersigned respectfully request to be absolved from any administrative

infractions and rest assured to faithfully obey legal orders from your office.

Dear Sir/Madam:

Greetings.

The undersigned police officer presently assigned at Manila Police District submitted for drug examination
last November 13, 2023. However, herein officer forgot to declare the medicine which was prescribed by
my physician for which I took in the morning of the said examination. On November 16, 2023, the
undersigned received a Memorandum stating that I was tested positive for illegal substance.

Thereafter, the undersigned submitted a request to MPD Forensic Unit for a Challenge Test on the
specimen which was later approved. However, herein officer was only given two (2) choices of the
laboratory to conduct the Challenge Test which they considered as the only two (2) DOH accredited
Confirmatory Laboratory in the Philippines.

In this regard, may I inquire if the laboratory of your bureau was accredited by the Department of Health.
Further, herein officer respectfully request that the said test can be accommodated in your forensic
section for impartiality and fairness considering the integrity of your bureau.
