SUBJECT : Request for DNA Examination

DATE : October 15, 2020

1. This pertains to the rape incident that transpired on October 15, 2020, at 1:00 o’
clock in the morning infront of Akhias Store located at 7 Armando Road Brgy Babag
Noliches Quezon,City , wherein victim one Mark Tabagan AKA “Kikay” 23 years old,
single, transgender, manager at Jollibee located at Quirino Hwy, Quezon City and
resident of 32 Armando Road Brgy. Bagbag Novaliches, was raped by the suspect
Tero Y. Kanto, 30 years old, single, tricycle driver, and a resident of 2 Armando Road
Brgy. Bagbag Novaliches.

2. In this regard, this office would like to request your Office to conduct DNA
Examination on the evidence samples purposely to determine whether the said
bloodstains are identical and came from the same person to wit:

a. One (1) pieces of blood swab (bloodstain sample) recovered from

the crime scene and marked as “MRG 1”

3. Further request to furnish this office a copy of the result of the said examination
which will be used in the investigation and to support the filing of appropriate case in
court against the above-named suspect/person.
