 CLUB/TEAM WAIVER AND PERMISSION FORM

Participants and their parents/guardian should be aware of the possible risks that are inherent in the
nature of the sports of futsal tournament. These risk include, but are not limited to, the potential for
accidents/injuries or illness while participating. Every attempt is made to minimize the existing risks
through proper sports equipment, safe facilities and sound safety practices. However, participants should
realize these risks cannot be eliminated.

By voluntary participation in the “7-aside Festival Cup 2019” we recognize, understand and agree:

1. That the competition carries with it potential hazards associated with the sport and acknowledge
that the “7-aside Festival Cup 2019” carries, no form of personal player insurance and is not
responsible for any case of injury, illness, theft or damaged to property,

Therefore, we assume all the risks of participation and hereby agree to indemnify and hold
harmless the PFF, LGU-Sto. Domingo, DUFC, its officers, directors, its affiliates, its appointed
officials, coaches or any entity involved in the Tournament for any injury/illness or loss of any
kind.

2. We affirm that all players are physically fit and have not been advised to not participate by a
qualified medical professional. We further certify that there are no health related reasons or
problems that preclude our player’s participation on this event.

3. In addition, we hereby agree and authorize the free use of our player’s name(s). image(s), video(s)
and/or voice(s) in any broadcast, telecast, print or any other account in any medium in any and
all publicity or promotional material relating to this event, without compensation. However, we
understand that the organizers will assure that all materials will be used in good faith and will not
be detrimental to the participant involved.
