1. INTRODUCTION

Driving is one of the greatest risks most likely faced by any CONTRACTOR
employee’s. To minimize the risk, the following Site Traffic Management Plan shall be
implemented for Vehicle Operations with a specific business purpose in all
CONTRACTOR facilities on the Project.

2. PURPOSE

The purpose of this Procedure is to specify requirements for Safe transportation of


personnel and equipment at site.

3. SCOPE

This Procedure applies to CONTRACTOR and SUBCONTRACTOR Personnel who


operate motor vehicles on the site either as part of their duties relating to an
assignment, or as a form of transportation.

5.2 Definitions

Mobile Phone Cellular and other mobile telephones or other similar


electronic communication devices which facilitate
both simplex (push to talk) and duplex (multi
directional simultaneous conversations), when those
devices are being used for voice communication, text
messaging, electronic mail, and other operations
requiring active manipulation of the device, including
operation of the devices in hands free or hand held
modes.

Motor Equipment Such as tractors, lawnmowers, forklifts, mobile


cranes, and fire truck are not considered Company
Vehicles for the purpose of this procedure. Accidents
involving such equipment will not be classified as a
vehicle accident

6. RESPONSIBILITIES

6.1 Line Management

Line management at all levels shall demonstrate active, visible leadership and
personal participation in all aspects of plan by:

- Ensuring that drivers under their control have the relevant CONTRACTOR
approved driving license before allowing them to drive vehicles.
- Emphasizing driving risks in all appropriate meetings.
- Promoting safe driving awareness to minimize night driving and the total
number of miles driven.
- Always searching for safer means of transportation other than driving.
- Ensuring that all CONTRACTOR and/or SUBCONTRACTOR vehicles
transport are equipped with adequate safety equipment.
- Insisting that everyone under their responsibility demonstrates similar
leadership.
- Ensuring that the CONTRACTOR and/or SUBCONTRACTOR vehicles
transport is maintained in accordance to the manufacturer's specification
and to ensure that drivers who drive on CONTRACTOR business meet all
requirements.
- Providing adequate resources to help drivers conform to the requirements
of this Plan, including driver training and a safe transport. With an ever-
changing business, particular attention must be given to the hazards of
driving in unfamiliar locations.

6.2 Driver

Any employee that drives a CONTRACTOR transport is responsible for:

 Drive safely and comply with applicable laws and regulation.


 Have a CONTRACTOR approved license prior to driving a
CONTRACTOR transport.

 Ensure all passengers comply with applicable CONTRACTOR regulation


on the CONTRACTOR transport, e.g. wearing a seat belt.
 Actively participating in pre-trip briefings, if applicable.
 Actively participating in pre-use inspection prior to operated vehicle
 Comply with the pre-trip plans and all other procedures detailed in this
standard.

7. PLAN

7.1 General

A driver or operator of vehicles or mobile equipment such as mobile cranes,


light vehicle, trailer, etc., entering or working at the construction site shall obey
this traffic rule and the specific CONTRACTOR and COMPANY Rules. At
public roads, the local traffic law and regulations shall be observed.

7.2 Vehicle

7.2.1 Vehicle Selection

The following procedures must be applied when choosing vehicles for


company business:

 Vehicles with convertible, removable, or no tops must not be used.

 Vehicle to be used on mud terrain must be four wheel drives (4WD).

 Vehicle shall be equipped with ABS brakes.

 The following equipment should be installed and securely fixed,


where appropriate, on light duty vehicle. As a minimum, vehicles
shall be equipped with the following:

 Head rests

 A basic radio type to receive alerts about Economic, Social and


Political conditions

 Air conditioner

 Solar film coating

 First Aid Kit

 Fire Extinguishers minimum 1 kg ABC class

 Safety belts for the driver and passengers.

 Tool kits to change the tire

 Suitable Spare wheel and tire

 Disable vehicle marker (e.g. Warning triangle)

 Flash light.

 (see Vehicle Inspection Check-List).

Where a risk assessment demonstrates that the risk of rollover due to


terrain, a vehicle type or work condition is higher than normal, a properly
engineered rollover protection device must be installed (internally or
externally).

7.2.2 Vehicle Condition

A qualified inspector from CONTRACTOR must inspect all equipment


used for jobsite transportation of personnel and equipment prior to their
use on site and at least quarterly thereafter. Records of all inspections
and maintenance shall be completed and maintained for review by
COMPANY.

Quarterly or periodic inspection shall be carried out by the mechanics


in accordance with the Inspection Procedure prepared by the Safety
Supervisor.

All vehicles in use shall be checked at the beginning of each shift to


ensure that the following parts, equipment, and accessories are in safe
operating condition and free of apparent damage that could cause
failure while in use: brakes (including trailers), lights/reflectors,
windshield wipers, defrosters, tires, horn, steering mechanisms, fire
extinguishers, coupling devices, seat belts, operating controls, and any
other safety devices.

All vehicles and construction equipment shall be well maintained and


ready for use. The SUBCONTRACTOR that owns the vehicle or
equipment has the responsibility for its inspection maintenance.

The inspection and maintenance records shall be submitted to and


retained by CONTRACTOR Safety Office during the entire construction
period.

7.2.3 Cargo

The cargo on the vehicle shall be secured with bundles or ropes to the
vehicle in order to preventives moving, slipping or over turning.

A cargo which is less than 1 (one) meter or more than the length of the
vehicle shall be tagged with red flags on the both ends of the cargo
during driving and parking.

7.2.4 Passenger

It is forbidden to transport workers by any mobile equipment (mobile


crane, fork lifts, trailers, etc.) or in cargo vehicles, such as trucks, trailer.

When a truck or the like is used to carry workers, passenger’s seats


secured to the floor and guard’s rails to prevent falls shall be furnished.

All drivers and a passenger beside the driver in a vehicle are required
to wear the seat belts furnished in the vehicle.

7.2.5 Convoys

For the large vehicle or non routine loads, an escort vehicle provided by
the CONTRACTOR should be used in front of and when necessary,
behind the vehicle or mobile equipment.

7.2.6 Vehicle Pass

Any vehicle entering the Project Site shall require a Vehicle Pass issued
by CONTRACTOR Safety Supervisor in accordance with the
CONTRACTOR SHE Plan.

The vehicle is working at the site, a permanent Vehicle Pass with a


validity term will be issued, and for visitor’s vehicle, a temporary Visitor
Pass will be issued.

The Vehicle Pass shall be placed in a prominent location, such as the


front window of the vehicle, when it is on the Site for ease of gate
control, and observation of operation, parking and traffic rule violation.

7.2.7 Speed Limit

Vehicle’s maximum permitted speed within the Site will comply with
COMPANY AND CONTRACTOR traffic rules.

All vehicles shall move slowly in congested locations and/ or poor


roads conditions due to weather or ground conditions.

All vehicle and mobile equipment shall stop at road corners and
gradually start after confirming safely.

Maximum Speed Limit at On-Site:

 CONTRACTOR will restrict vehicle movement from work site


exclusively for movement of materials and equipment.

 Speed limited at work area shall be determined based on access


road condition, activities at the location, etc.

 The maximum speed allowed on site transportation is not more than


40 KPH

Maximum Speed Limit at Off-Site:

Drivers shall:

 Reduce speed according to prevailing road, traffic and


environmental / weather conditions

 Be on the convoy and follow the instruction on speed set on different


point highlighted by escort team.

 Not exceed any signposted speed limits

 Speed limits for driving at night shall be 60 KPH maximum for all
blacktop roads outside the Construction site and 40 KPH maximum
for all graded roads within field location

 Speed limits for driving off-road shall be 50 KPH maximum

7.3 Driving Between Field Location and Residence

 CONTRACTOR will provide transport for their employees between their


provided accommodation and their place of work in the field location.
Employees travelling from own residence shall be picked up from dedicated
Bus terminal near to project locations. Such transport shall fully comply with
requirements of this procedure

 CONTRACTORs shall strongly discourage their employees from using their


own vehicles for this purpose.

7.4 Driver

7.4.1 Training

 All CONTRACTOR and/or SUBCONTRACTOR transport drivers


shall be trained in compliance with the Safety Training Document
Procedure, Document BLA-FTP-H-PR-00-005 and Defensive
Driving Training.
 Operators of motor vehicles and other equipment must be licensed
to operate that equipment.
 CONTRACTOR must ensure that equipment operator’s had
received training and certifications from third party organizations.
Otherwise the CONTRACTOR must develop and implement
training and testing program for authorization of equipment
operators.
 Copies of the above documentation must be retained on site by the
CONTRACTOR.
 CONTRACTOR must also issue proof of qualification to each
operator, if applicable.

 Operators must have this proof of qualification with them at all times
while operating that equipment.

7.4.2 Work Hours Plan

Drivers must not operate vehicles unless appropriately rested and alert.
In particular:

 Driver shall have adequate rest time prior to drive. A process shall
be in place to check prior to each journey whether the driver is fit to
drive.
 Maximum duty hours for driver should be defined by site line
management based on applicable regulations and standards.
 Drivers must advise management when they have a disability or
condition that could prevent them from driving safely.
 Drivers shall have the right to refuse to drive when they feel that
they are not fully rested or alert.
 Drivers shall be informed on how to identify driver fatigue and
alertness, and means of dealing with them.

7.4.3 Drug and Alcohol Test

Drivers shall not operate a vehicle while under the influence of alcohol,
drugs, narcotics or medication that could impair the operator's ability to
safely operate the vehicle.

Driving a CONTRACTOR transport while under the influence of alcohol


or any drugs or narcotics, is strictly prohibited and subject to disciplinary
action which may include termination of employment.

Post accident test of alcohol or any drugs or narcotics, if applicable,


should be carried out to the driver after vehicle accident occurred.

7.4.4 Fitness for Driver Medical Exams

All persons employed as drivers and persons regularly driving on


CONTRACTOR business must be medically assessed to ensure that
they have functional capacity to operate a vehicle safely (refer to

7.4.5 Driver for Personal Protective Equipment

Standard safety equipment is required when on site (Exception: lunch


rooms, bathrooms, designatedoffices), and includes:

 Hardhat
 Eye protection with shield
 Ankle high leather work boots with hard toe protection
 Long sleeved, tropical weight coveralls
 Appropriate gloves (When handling materials/using equipment)
 Hearing protection (When around noise levels above 85 DBA)
 Respiratory protection (When working in areas that could produce
respiratory concerns)
 Other special protection equipment as required

In general, comply with and wear the PPE designated in the Project
SHE Plan, Document No. BLA-FTP-H-PR-00-018; Personal Protective
Equipment.

7.5 Incident Management

7.5.1 Mechanical Issue or Breakdown

Servicing and repairs of vehicles shall comply with the following:

 The service intervals, tasks, component replacement and fluid


change requirements specified by the vehicle manufacturer shall be
adhered to;
 The work shall be carried out in accordance with the manufacturer's
instructions at an approved supplier;
 Only components supplied by the vehicle manufacturer or a
reputable components supplier shall be used;
 Fluids used shall riieet the vehicle manufacturer's specification;

 Checks shall be carried out on additional equipment fitted to meet


CONTRACTOR requirements, e.g. fire extinguishers, first aid
boxes, etc.
 A Vehicle Service and Repair Record is to be maintained, listing all
routine servicing and rectification work carried out on the vehicle,
including:
 The manufacturer's recommended service carried out;

 Repair work carried out including components replaced;

7.5.2 Road and Traffic Accident

 Vehicle-related incidents will be reported per CONTRACTOR


Procedure of Occupational Near-Miss / Accident / Illness Report
Document No. BLA-FTP-H-PR-00-008.

 Vehicle-related incidents are subject to a Management Safety Review


within 48 hours.

7.5.3 Rules Violation

Any drivers or operator who seriously or repeatedly violate on site traffic


rules and the local traffic rule while driving on the roads shall be subject
to remove from the site by Site Manager / Construction Manager’s
discretion.

7.6 Other Vehicle Safety Procedures

7.6.1 Traffic Regulation



Drivers must obey all state, city and rural traffic regulations and
demonstrate effective defensive driving techniques. Appropriate
driving standards must be observed.

Drive slowly where rocks and gullies prevail.

Avoid “back-out” situations. Attempt to “back-in” to Parking spaces
wherever possible.

Always rear parking, first move forward of the vehicle.

Head Light and/or rotary / hazard light ON while driving.

No transporting people on the bed of pick-up.

7.6.2 Wet Weather Driving

The following requirements shall be observed with regard to driving in


Wet weather conditions:

 Precautions addressing driving in adverse climates, foul weather


conditions, difficult/ challenging locations and driving at night shall
be established.
 Applicable precautions shall be identified during Journey Planning
and recommendations shall be made to avoid risks/hazards.
 Drivers shall be informed of the precautions and recommendations
prior to the start of the journey.
 Employees shall not set out on any journey in wet weather
conditions unless absolutely necessary and then only after the
appropriate approval has been granted.

7.6.3 Road Flooding

 All vehicles shall use the designated entrance and road for access
to work location, except in case emergencies.

 It is forbidden to obstruct any road with vehicles, materials, etc, or


to park vehicles near the entrance to buildings, or near fire stations
and fire hydrants.

 Whenever possible the designated parking area shall be used to


park vehicles.
 When a vehicle must be parked on the road, parking sign or
barricades shall be placed at the front and the rear of the vehicle.
 When parking a vehicle on roads during the night, reflective markers
shall be placed. When a driver leaves the vehicle, they shall turn –
OFF engine, apply the parking brake and lock the door.

 All vehicles shall be reversed parked only

7.6.4 Civil Unrest

 As recommended by the Management Safety Review Team, the


driver(s) involved in the vehicle related incident may lose their
driving privileges.
 Due to interface with the general public, extra precautions are
required to restrict access into the project area.
 This will be achieved by the use of signage and the issuing of
public information bulletins to notify the public that beyond a
designated location is a project area and access is restricted to
Project personnel and contractors only.
 Where appropriate, additional warning signs or personnel will be
used to control traffic flow.

7.6.5 Smoking

Smoking in vehicles or the cabins of mobile equipment is strictly


forbidden. Smoking is permitted at permitted location only.

7.6.6 Motor Cycle

The use of motorcycles is prohibited in the Work Site. No motorcycles


all around in field roads and/or ROW facilities road.

7.6.7 Refueling

 The refueling location is fenced around and clearly indicated with


warning sign such as “TURN-OFF engine” and “NO SMOKING”.

 Equipment shall be TURNED OFF during refueling.

 Filled cans and the empty cans shall be stored separately. The
empty cans should be removed from the area as soon as possible.

 Adequate fire extinguishers shall be furnished at the location.

7.6.8 Driving Out-Side of the Site

When outside the Site, all drivers have to observe and comply with the
traffic regulations specified by the Kepolisian Negara Republik
Indonesia.

7.6.9 Night Driving

A prominent national safety council reports that mileage-based death


rates are four times higher at night than during the day. Night driving
shall always be minimized through prior planning.

All night trips shall be approved by the line management before they
begin. Wherever night driving required, a site-specific night driving
requirement shall be established basedon the risk assessment.

7.7 Operation

 Mobile equipment may carry in the cab only as many employees as there
are factory-installed seat belts.
o If equipment does not have additional factory-installed seat belts in the
cab, then only the operator should be allowed to ride the equipment.
o Under no circumstance, no permitted to ride the bed of trucks.
 Seat belts shall be worn by operators and all passengers when seated at
the controls.
 Equipment regularly used to transport personnel must be equipped with a
safe means of mounting or dismounting. Employees may mount or
dismount only when the equipment is fully stopped.
 Operate equipment according to the posted safe speed limit.
 All engines of vehicles and heavy equipment must be turned off during
refueling operations.
 All equipment left unattended at night shall have appropriate lights or
reflectors, or barricades equipped with appropriate lights or reflectors, to
identify the location of the equipment.
 The parking brake must be set whenever equipment is parked.
 Equipment parked on an incline must have the wheels chocked.

 No employee is allowed to rest directly in front of or behind of any piece of


mobile equipment.
 All raised hydraulics shall be securely blocked while repair work is being
accomplished to prevent their release and subsequent fall.
 Do not back-up any vehicle or equipment when the view of the rear is
obstructed unless your spotter/flagman signals it is safe to do so.
 A face shield and proper gloves are required while handling or recharging
batteries.
 Loads extending beyond the bed of a truck or wagon are to be flagged or
marked with red flagging and/or clearance lights.
 Tools and material shall be secured to prevent movement when
transported in the same compartment with employees.

7.8 Environmental Consideration

Prior to transportation of materials and equipment to work sites,


CONTRACTOR Environmental Officer shall carry out an environmental
screening. This screening consists of study of available existing routes to the
construction sites and selecting the routes, which has least overall
environmental impacts. These routes will be identified on the Site Traffic
Management Plan. All drivers will be informed about the designated routes and
the necessity to comply with such routing directions.

Prior to start of any transport operation, the environmental Officer shall


organize environmental awareness training to all drivers and discuss the
requirements of the Environmental Management Plan, Document No. BLA-
FTP-H-PD-00-008,

Fluid systems must be maintained to prevent leakage. If a leak or spill occurs


in a hydraulic system, the spill must be cleaned up immediately in accordance
with governmental regulations.

All vehicles exiting the site must pass through the wheel washing facility and
the site security compound.

The CONTRACTOR Safety personnel will monitor vehicle movements and


ensure all drivers adhere to the approved routes. Violations will be recorded
and brought to the attention of concerned supervisor for immediate action.

7.9 Driving During The Month of Ramadhan

 During the month of Ramadan every effort shall be made by supervisors to


limit driver duty hours whenever practical.

 A second driver shall be considered, and if available utilized, on journeys


which take over 6 hours.

 Drivers shall be sufficiently rested-if a driver has not slept at least 8 hours
in the previous 24hours he shall not be sent on a journey.

 Arrival at destination must be before the onset of darkness.

1. Safe Driver
a) Complete pre-start checks and keep the vehicle clean.
b) Plan the journey.

c) Before setting out on any trip, you must be fit, alert and have the following documents in your
vehicle:
d) Your driving license
e) Company’s authorization letter (Driving Permit).
f) Move only after everyone’s seat belt is fastened. Any unsafe passenger behavior should be
reported immediately.
g) Rest: half an hour after 3 hours driving.
h) Headlights on at down & dusk – at least 30 minutes before sunset (dusk) and 30 minutes
after sunrise (down), and during poor visibility. Use horn to warn not to annoy.
i) Use phone and radio only when stationary. Keep your radio/phone talks to minimum. Never
initiate or answer calls while driving.
j) Drive at safe speed! Follow the law 100%.
k) Keep the distance. Respect passenger and all road users including pedestrians.
l) Report all collisions and faults promptly, specify all the details for effective investigation.
2. Safe Passenger
a) Always wear seatbelts- be certain they are accessible and in good condition.
b) Secure your cargo-loose objects can become dangerous projectiles.
c) Be on time – avoid rushing the driver
d) If in doubt, confirm that the vehicle has received a due daily inspection and has safety
equipment in board.
e) Warn the driver if he is traveling too fast for road/weather conditions, or shows any non-
compliance with the safe driver.
f) Be prepared to provide clear directions/instructions to the driver when/if necessary.
g) Assist the driver when maneuvering/backing/parking in hazardous or confined area, as
required.
3. Vehicle Loading
a) Vehicle loads must not exceed the weight limitations indicated on the vehicle registration
document.
b) Vehicle loads must not exceed over the sides of the vehicle.
c) Any load which extends beyond the rear of the vehicle must be marked with red flag.
Vehicle loads must be properly secured. Loose tools or equipment must not be carried inside the
passenger compartment.
