B. Brief statement of the problem

She was brought by her grandmother for the first time at the center last April 23 2008 because
of sleeping difficulty, poor appetite, having phobia and allergy to sea foods.
II. Intake Interview

A. Case History

1. Family History
a. Parents​- His father is 45 years old, a high school graduate, farmer and described by her
daughter as Kind , Strict, Hardworking and Responsible Father to his children. While
her mother is 37 years old, a full time housewife and described by her daughter as
lovable and caring mother.
b. Siblings ​– her brother is 13 years old, a grade 11 student. Her youngest brother is 8
years old and her sister is 2 years old.
c. Large Family Circle
d. Physical surrounding​- she lived in her maternal grandmother’s house in Isabela
located at Salvacion , Alicia, Isabela . It is a simple house with a three rooms.
2. Personal History
Her grandmother tell information about her prenatal and natal; and childhood
development
a. ​Early Development​- when she was a baby her mother had a regular check-up with
her OB Gynecologist and took multivitamins.
She is temporarily taken cared by her mother as she breastfeed for a few days from
birth followed by formula milk. Her mother left her with her grandmother when
her mother went abroad due to financial problem.
Disciplined was installed primarily by her grandmother at the age of five. She use to
spend her time playing with other kids, during her childhood she have an asthma
at the age of three, hospitalized several times in a year from three to eight years
old . Her asthma lessened when she was eight . No history of seizure disorder ,
head trauma or any surgical operations as noted.
b​. School History​- at the age of five ,she started schooling in Salvacion, Elementar y,
School with no separation anxiety encountered. In high school , she enrolled at
school of Our Lady of Atocha in Alicia Isabela. Based on her academic
performances her interest is on Filipino, and practical arts but had difficulties in
mathematics. She graduated with an average rating.
In college she took up psychology. It is not her first choice though.
c. ​Sex History​- she had her first menstruation at the age of fifteen and she had her first
crush during those days.
d. ​Major Experiences​- she is doing well in her college life. Until there is a sudden
behavioral oddities such as difficulty in sleeping, overly concerned with her
relatives reactions towards her, becoming suspicious and started to isolate
herself from everyone are observed.
This person was a jolly and joyful sister, suddenly something change because of a
tragedy, the result of tragedy to her is, she become aggressive , she can’t control
herself and become wild when she is in crowded and noisy places. Sadly there is
no diagnosis in what behavior she has now, nor her family can’t afford to bring
her to specialist.
e. Estimate of self and the World
She perceive the world as cruel that she reports “pakiramdam ko wala akong kwentang tao”
She related that she only became depressed when she become rejected.
III. Physical and Behavioral Observations
Lalaine is about 5 feet tall, her long hair which is about shoulder’s length that reaches her back.
She is usually seen wearing T-shirt, pants, and rubber shoes. She has a good orientation of date,
place and time. She is also aware of the current events and can also re mote and recall
the past.
During the interview, Lalaine spoke softly and with reservation. She was very evasive and
cautious while answering the questions that being ask.
During the interview lalaine keep on looking at the ceiling from time to time.
IV. Testing Behavior

Lalaine was fairly cooperative during the administration of the psychological tests. She was
somewhat suspicious about the reasons for taking the test. Nevertheless, she followed
instructions carefully with very little supervision. She finish the test ahead of time.

V. Test results and Interpretation

Test Administered: House-Tree-Person Test

Date Administered: April 26,2017

Based on the test results administered, lalaine’s test shows that she exhibit a behavior that is
somewhat cautious because of her being self-protective. She is concern over interpersonal
relations and artificial fixations. This person is somewhat insecure and timid. Lalaine tends to be
aggressive, and depressed. However ,the result of the test shows that she is somewhat calm
and has a relatively stable personality.
