A. Family History

Her parents are Mercy S. Gracilla, a 48 year old businessman and Cesar H. Gracilla, a 49 year old
marine engineer. Her father works abroad. The client has one sister, Ivy Blanche, a 16 year old 4 th year
high school student. And also has one 13 year old brother.

B. Physical Surroundings of the Youth

During her early childhood, she stayed with her nanny most of the time because her mom has to
work and her dad has a contract abroad. She started schooling at the age 4 where she met new friends.
And on the latter run, she started playing in the streets with her neighbors.

C. General Home Atmosphere

Her family gets along very well. They eat and pray together. Despite the distance, she is still very
close to her father whom she considered as very sweet and loving, a good family man and a good
provider. Same goes to her mother whom she treats like her best friend. She shares an open

relationship with her mother. She loved her mother for being supportive and understanding. They
usually spend time shopping or doing household chores together.

And as the eldest child in the family, she took the responsibility of taking care of her siblings
whenever their parents aren’t home. Her parents taught her to be independent and to take
responsibility for herself.

D. Personal History

 Developemental History

Her development history was considered to be normal. She was conceived on 28 th of October on
the year 1988 at exactly 5:30 in the morning.

 School and College History

She started studying at the age of 4 in Gregorio Araneta University Foundation (now, De LaSalle
– Araneta). The client later on pursued high school in St. Mary’s Academy of Caloocan City, an exclusive
school for girls and took college in San Beda College after.

 Peer Relationships

She had a lot of friends in and out of the school. They spend time together doing what most
friends of her age do. She treasures all her friends from elementary to college.

 Employment History

She was never been employed and has no experience in work.

 Romantic Relationship History

She had her first boyfriend when she was in her first year of college. The relationship lasted for
only six months due to their differences that caused her to feel inferior and unconfident. The day after
they broke up, her recent girlfriend was introduced to her by her team mate that led her to regain
confidence and high self-esteem. She considered their relationship of 2 years and 11 months as very
healthy for her.

 Sex History

She does not have any experience on sex because she thought that it was not the right time for
her yet. Although, she experienced her first kiss with her first boyfriend and nothing followed after that.
She was fond of kissing and hugging all those whom she loved but nothing more intimate than that.

 Aggression History

She considered herself aggressive when it comes to taking care of her siblings. She felt the need
to be tough because she was the eldest of them. Although sometimes, she loses her temper and hits
them.

 Medical and Health History

When it comes to her medical history, she had experienced a lot of accidents during her early
days. When she was a baby, her mother often took her to the hospital because she is suffering from
difficulty to breathe. When she was in kinder, she fell from the back of an owner type jeep at
Balintawak. She was also hit by a bicycle that caused her to be rushed in the hospital. And she often fell
from the stairs due to activeness. From this point on, she never had chicken pox.

 Alcohol and Drug Hisotry

The client started drinking when she turned 18. From then on, she has become an occasional drink.
She has no history on drug intake.

 Major Experiences

While she was in her elementary, she was fond of engaging in extracurricular acrivities such as
dancing and playing table tennis as a varsity in school. She enjoyed spending time with her friends
and loved ones. She values her experience of loving and being loved. Her major experience of
depression was when she had to transfer to St. Mary’s Academy for her high school. She felt so sad
about leaving her friends and it took her three years to adjust to the environment of an exclusive
school for girls.

 Aims and Aspirations

The client aims to finish her studies on time and be able to land on a stable job. She also would want
to pursue a master’s degree. Aside from these, she is hopeful to get her own dream house and car as
well.

 Estimate of Self and World

She sees herself as a beautiful creation of God and as a caretaker of the world we live in. she values
time, life and her loved ones. She has a very positive outlook in life and believes that life is filled with
experiences, explorations and wonderful memories.

IV. Physical and Behavioral Observations

She was very cooperative in giving out information about herself. She seemed eager to take the
tests and to see the results. She is very patient and directly asks questions if the instructions weren’t
clear. It goes to prove that she is listening very well.

V. Testing Behavior

She did not show any consistent mannerisms during the testing. She was relaxed. The client was
focused and she eagerly does what she is asked to.

VI. Test Results and Interpretations

 Guilford-Zimmerman Temperament Survey (Novemeber 20, 2007)

The results showed that she has the tendency to be impulsive in making decisions however she is
also very reflective. This trait would make her analyze the pros and cons of her decision afterwards. It
also revealed that she is thoughtful, energetic and has a strong drive on the things she enjoy doing.
