the farmer-beneficiary of a parcel of land particularly known as Lot 1083-D Psd-03-029945, with
an area of 7,328 sq.m., more or less, situated at Gabihan, San Ildefonso, Bulacan embraced by
Transfer Certificate of Title No. CLOA-T-2929 entered in the Registry of Deeds of Guiguinto,
Bulacan on November 28, 1991.

It is further certified that said lot has been fully paid by the farmer-beneficiary under O.R.
No. 20271828 dated March 21, 2016.

This certification is issued upon the request of above-named FB for the purpose of
cancelling the annotation at the back of the TCT.
