AUTHORIZATION LETTER

I, Rudy T. Fernandez, married, with residence/address at 04 Sitio Ruby Ext., East Fairview, Q.C.
and who is presently an Overseas Filipino Worker based in Saudi Arabia do hereby authorize Riza Joy
Fernandez, my daughter, of legal age, single, and with residence same as mine to verify if there’s a loan
applied. If there is, and the authorize name is not my daughter, kindly cancel it. I’m no longer authorize
anyone except my daughter to apply the loan on my behalf. And if no loan existed, I will authorize then
my daughter to file my loan application duly signed by me, to sign and receive the Disclosure statement
under my loan application and to receive the check in my behalf.
