Thank you for agreeing to complete a reference form for the Nursing and Mid-
wifery Council (NMC) registration process in the UK.

Please note there are a number of guidelines that you should follow to ensure
the reference form is accepted by the NMC.

–– Reference must be completed by a senior member of staff who would have

supervised/managed the candidate in a clinical setting.

–– References must only cover NURSING experience. It should not mention

midwifery if the candidate is applying as a registered nurse to the NMC. Discover us

–– If the nurse is currently still working with you, an end date will still need to Sandyford

–– Please ensure to sign, date and stamp the reference form. ttmhealthcare.ie

–– If you do not have a stamp, please provide a letter confirming you do not
have a stamp on Company Headed Paper. This must also be signed by you
and accompany the reference or you can use a dry seal as another alterna-
tive.

–– The stamp required for the reference must be a company stamp and not an
individual persons stamp.

–– If the candidate was not in a ward setting (for example a Public Health
Nurse) it is extremely important to highlight that the nurse still worked as a
registered nurse.

–– If you are providing a reference for the candidate as a Health Care Assistant
please advise to the candidate that you cannot provide a reference. Refer-
ences MUST cover post registered clinical nursing experience only.

Please ensure that all the information on the reference is clear and accurate. If
for example the dates of employment are not matching the dates provided to
the NMC by the candidate the NMC will not accept the reference and it will be
requested again.
