Rationale: Correct catheter care decreases risk of infection and
contributes to the clients overall comfort.
1. Assemble your equipment :
Basin of water and mild soap or cleaning solution, wash or gauzed
pads, paper or plastic bag for waste, disposable gloves
2. Wash your hands.
4. Tell the client you are going to give him catheter care.
5. Position the client on his back so the catherter and urinary miatus
are exposed. Put on your gloves.
6. Wash the area gently. Do not pull on the catheter, but hold it with
one hand while wiping it with the other.
7. Observe the meatus for redness, swelling, or discharge.
8. Wipe away from the meatus. Wipe from the meatus to anus
12. Apply lotion or powder in small quantities to the thighs. Ask your
supervisor if this area should be kept dry or moist.

13. Make the client comfortable.
14. Put on gloves. Dispose of the dirty water into the toilet. Clean your
equipment and put it in its proper place.

Equipment for mouth are following this procedure, tissues and basin or
specimen container, plastic bag for waste
2. Wash your hands
4. Tell the client you are going to help him with deep-breathing
exercises.
5. Direct the client to breathe in deeply through his nose.
6. Direct the client to blow out through his mouth with his lips
"pursed", as though he were blowing out a match

9. Put on gloves and take the bedpan or urinal into the bathroom
10. If the client is on I&O. Pour the urine into a clean measuring
container and note the urine amount on the I&O sheet.
11. Pour the urine into the specimen container. Fill it 1/3 full.
12. Put the lid on the container. Wipe off the outside of the container.
Secure the label to the container.
13. Pour the urine remaining in the bedpan, urinal, or measuring
container into the toilet.
14. Clean and rinse the bedpan, urinal, oreasuring container and return
to their proper place.
15. Remove gloves and wash your hands.
16. Make a notation on the client chart that you collected the
specimen, the time, and anything you observed about the client during
this procedure.
17. Store the specimen in the correct place before it is taken to the
laboratory.

5. Put on gloves.
10. Insert the urine into the sterile specimen cup. Take care not to
touch the rim or inside cover of the cap ( prevents contamination of the
specimen).
11. Unclamp the tubing and straighten it out. Be sure that the urine is
now freely flowing.
12. Apply the correct lab label the specimen cup and bag according to
policy.
13. Report the result of the procedure to your supervisor.
15. Make a notation on the client chart that you collected the
specimen, the time, and anything observed about the client during this
procedure.
16. Store the specimen in the correct place before it is taken to the
laboratory.
