• Certification as a candidate for Grade 12

graduation in the current school year
• Copy of K-12 Report Card
APPLICATION or Certified True Copy of Grades
REQUIREMENTS • A grade average of at least 80% or better in the
third grading period at the time the applicant
is taking his/her Grade 12
• Three (3) 2 inches x 2 inches ID photos
