ANNEXURE I : SPECIAL CONDITIONS OF CONTRACT (SCC)


The following are the Special Conditions applicable to this contract which shall be read in conjunction with
the General Conditions of Contract, Specifications, drawings and/or any other document forming part of the
contract. In respect of terms and conditions, not explicitly provided in this Special Terms and Conditions,
provisions of General Conditions of contract are applicable."

A) Technical Terms & Conditions


1.0 JOB COMPLETION TIME:
For GC Unit: The job shall be completed within 10 days from the date of issue of front clearance/ first work
permit.
For LDPE Unit: The job shall be completed within 03 days from the date of issue of front clearance/ first
work permit.

2.0 Working hours pattern:


Working hours pattern shall be on round the clock basis in shifts of 12 hours each.
Contractor shall carry out the work on Sundays and Holidays as per RIL requirements.

3.0 MOBILIZATION TIME:


3.1 Contractor shall mobilise all resources including tools and tackles and manpower resources within 4
days from the date of issue of work order/LOC.
3.2 Contractor shall mobilise resources at the earliest but not later than 13/07/11.

4.0 BROAD SCOPE OF WORK/SUPPLY:


I) Scope of WORK General :
1. The work shall be performed in accordance with the applicable codes and practices.
2. Contractor shall make himself fully conversant with the locations and the type of job to be carried out
therein so that he clearly underst ands the scope of work and assess the requirement of resources required
to complete the work in scheduled time. He shall contact the respective Engineer-in-charge for this purpose.
3. The Contractor shall prepare plan for execution of jobs and get the same approved by
Engineer-in-charge. The contractor shall submit progress report at specified intervals and shall be
responsible to ensure the specified progress.
4. OWNER reserves the right to off load part or total quantum of the job depending upon the exigencies.
5. If the quality of job carried out is not to the satisfaction of the Engineer in charge, whose decision will be
final and binding on the contractor, the same job will be got done by employing any other contractor and the
charges paid to other agency plus 20% will be recovered from the contractor's bill.
6. In the event of non availability of consumable and other materials (under OWNER scope), the contractor
shall arrange to procure and supply the same on specific request made by Engineer in charge and the
contractor is entitled to charge the actual reasonable market cost of the same plus 10% (Ten percent) for all
such supplies. The purchase should be done from a sales tax /VAT registered party only. The claim
should be supported by the original copy of the cash memo with gate entry stamped as per GCC.
7. The contractor shall work as per instructions & priorities given by Engineer-in-Charge.
8. Execution of job includes execution of all preceding and connected jobs.
9. The contractor would enter into secrecy agreement with OWNER with respect to all drawings, design
documents / information that they may come across during course of execution of the job.
10. The contractor should comply with all applicable laws including labour laws, regulations, ordinances etc

of the state / province pertaining to the work.

II) Scope Of Work Technical:


The scope of work includes but not limited to:

1. To carryout filtration of Lube oil/Seal units of CG Compressor (10,500 litres); C2R & C3R Compressors
(9,000 litres), Expander Compressors (1,500 litres) Units of Gas Cracker Plant and 30P41 lube oil console
(2,000 litres) of Hyper Compressor Unit of LDPE Plant. Filtration to be done NAS 6 or lower by deploying
sufficient nos. of trained personnel and resources at Owner's site for round the clock working on all days
(Sundays/Holidays) .
2. Contractor shall mobilise at least 2 units of Filtration Systems with all the required material viz; Brass
Pre-Filter with 600 micron stainless Steel cartridge, Tushaco brand Rotary Gear Pump, 0.5HP single phase
220 volts motor 1450rpm, fitted with Complete Electrical Panel, Pressure Gauge, Pressure cut-off switch,
Hour meter, hose (6m/unit) and contamination patch testing kit.
3. To mobilize adequate no. of filters to ensure continuous operation of filtration system.
4. To carryout Oil purity test using patch testing method.
5. To mobilize the filtration unit with all accessories, consumbles from Contractor's warehouse to Owner's
site and back.
6. To arrange mob-demob of crew from Contractor's works to Owner's site and back.
7. To carryout all connections required to establish circulation loop of oil reservoir and filtration unit.
8. To &Fro arrangement for their men & material.

III) General Scope:


1. Attending job as per SOR.
2. To Carryout the jobs in a manner as per the instruction of Engineer- in - charge. Job to be performed at
all heights and locations.
3. To take due care of items while handling and shall be liable for recovery for the loss / damage occurred
due to mishandling.
4. Jobs done by contractor shall be inspected by Reliance as per ITP/ Instructions of Engineer-In-Charge. If
any non-conformance is observed, contractor will attend/ rectify/ repair the same free of cost.
5. To mobilize sufficient resources including all hand tools / equipments, as required for the job based on
the target date of completion. All the lifting tools & tackles / measuring instruments used by the contractor
shall be duly tested & certified by authorized agency. Copies of all test certificates shall be produced for
checking by RIl before commencement of the job.
6. To arrange for collection of the material as per the requisition made by the Engineer In-charge from the
stores/ designated area including loading and unloading. Materialsinstalled shall be strictly as per
specification.
7. All other jobs covered in the tender document / scope of work.
8. Insurance of contractor's employees, tools & tackles & equipments .
9. Contractor to follow all safety norms as per present HSE norms indicated in GCC and as per revised HSE
norms from time to time.
10. To ensure that the work planned by Engineer-in-charge is completed in schedule. In the event of back
log, the contractor to increase the resources to make up for the unfinished work and to work during
extended hours or round the clock without any extra cost to OWNER.
11. Contractor to provide safety appliances like dust masks, safety harness, ladder, safety shoes, helmet,
hand gloves, safety goggles, PPE, rain gears, Overalls etc. to their personnel working inside the Complex at
his cost and should adhere to safety codes as given in General Conditions of the contract.
12. Contractor to maintain proper record of his tools & tackles, equipment, machinery and any other items
issued to him from owner's stores, with proper gate entry & shall provide access to Engineer In charge for
its inspection as and when required.

13. All materials, auxiliary equipment and consumable required for doing the job including operation of
auxiliary equipments.
14.Reconciliation of Free Issue Material : Not applicable for this job.

5.0 RIL's scope of work and supply:


5.1 Power, Water, Compressed air required for the job will be in RIL Scope of Supply.
5.2 Providing Job & material specifications, Drawings etc.
5.3 To provide additional storage reservoir if filtered oil is to be stored separately.
5.4 To certify the purity of the filtrate.
5.5 To provide P&M like fork lift/ hydrafor Unloading/Loading and shifting of Filtration Unit and its
accessories within the Plant limits.
5.6 To fabricate flange/fittings/clamp at suction & discharge of filtration unit required to connect with Lube
Oil sump. Isolation of lines.
5.7 Inspection & Certification of Elect Equipemnts.
5.8 Issue of valid work permit on round the clock basis.
5.9 To get the work front available.
5.10 Flood lighting and hand lamps at the job site.
5.11 Free electricity (including 110 V supply), air & water for the job at designated locations at utility service
points only.
5.12 Lodging & boarding at Madhuvan guest house free of cost.
5.13 Local transportation shall be provided to contractor free of cost.
5.13 In case of emergency only first aid medical facility shall be provided on chargeble basis. RIL will have
no liabilities towards injuries caused to any contractor's personnel in case of any accidents.

B) Commercial Terms & Conditions


1.0 PRICE BASIS:
1.1 The agreed rates are inclusive of all taxes & duties except Service Tax, which shall be paid extra.
1.2 SERVICE TAX:
Service Tax at applicable rate (Presently, Service Tax 10%, Edu. cess 2% of ST & SHE cess 1% of ST)
shall be paid extra on actual billing amount on submission of copy of PAN based Service Tax registration
certificate for the specific category of service and invoice in duplicate containing following details at the time
of raising your claim.
a) Serial No.
b) Name, Address and the Registration Number of the Service provider.
c) Name & Address of the persons receiving taxable service.
d) Description, Classification and value of taxable service provider.
e) Service Tax payable thereon.
f) Category of service for which registered.
g) Description of service provided.
h) Value of service provided.
i) Service tax amount paid or payable (Service Tax, Education Cess & SHE Cess amount should be shown
separately in invoice).
j) Period of Service from _______ to _______

Service Tax applicability will be considered only if a copy of PAN based Service Tax Registration Certificate
indicating Service Tax Registration Number and the Services for which registration was done is submitted
by the Bidder.

1.3 Contractor has to provide Service Tax Invoices separately showing the service tax element, if service tax
is applicable.

1.4 The rates offered by you shall include following:


-Cost of labour, tools, tackles & supervision and any incidental expenses thereon.
-Cost of transportation, medical test of contractctor's workmen.
-Rates shall be valid for all heights and locations.
-Cost of auxiliary equipments, consumables and all operations for completing the job
-Any other cost related with the job execution shall be included in order to have one point responsibility on
you.

2.0 PAYMENT TERMS:


2.1 No advance payment shall be made.

2.2 Separate Bill Invoice in duplicate as per given format on contractor's original letter-head has to be
submitted for scrolling in F&A Dept. alongwith measurement sheets duly certified by Engineer-in-charge.

2.3 Invoice shall be submitted exactly as per the original work order in line with the line items with actual
quantity executed. Additional quantity (other than WO) shall be claimed only after the issue of amendment
to the work order. Also where the rates are not available but the jobs are executed as per the instructions of
Engineer-in-charge, contractor shall submit the invoice only after the issue of Amendment to original work
order. Accordingly payment shall be released in two phases i.e based on Original WO & based on
Amendment to original WO.

2.4 If the job execution is less than 20,000 litres of oil


filtration then, a minimum payment of Rs.3,80,000/- shall be made
for 20,000 litres of oil filtration.
If job execution is more than 20,000 litres of oil filtration
then, payment shall be made as per the actual job execution and
certification by RIL Engineer-in-charge.
2.5 MODE OF MEASUREMENTS: JOINT MEASUREMENT SHEET(JMS):
Concerned EIC will maintain a record of measurements for each deployment. After completion of
job,immediately vendor shall prepare & submit JMS in the website :http://scm.ril.com/. The JMS is to be
certified & approved by EIC and QSD.

The vendor shall submit the invoice along with certified JMS copy, as per the terms of the contract within 14
days from the date of completion of job directly to the Finance Department for scrolling and release of
payment. While submitting the Invoice the Vendor shall attach a copy of certified joint measurement sheet.

2.6 Payment shall be made within 30 days from date of submission of bill for scrolling and job certification
by EIC after completion of job, through E-Payment only as per the payment methodology given at 2.7
below.

2.7 Payment Methodology:


Payments shall only be released by e-payment through the designated banks i.e. HDFC Bank/ICICI Bank or
Citibank,

a bank neutral system namely RTGS and NEFT.

2.8 Submission of PF challan copy and WC policy for obtaining "Clearance for Statutory Compliance" from
RIL Contract Labour Cell, shall be attached with Bill Invoice.

3.0 PENALTY FOR DELAY IN COMPLETION OF JOB:


IWaived off for this job.

4.0 As per statutory requirements, you shall submit the following details:

5.0 You shall be responsible for any losses incurring to the company or its agencies or its employees or its
property by way of negligence of safety/security rules on the part of contractor or its labour or its agency
during the execution of the contract or any damage/non returning of the issued items, covered under RIL
scope of supply, damage of any components while assembling or loss of any part including hardware due to
negligence of contractor, the RIL Engineer-in-charge will decide about recovering the cost of such items at
the prevailing price from the contractual bills.

6.0 SECURITY DEPOSIT:


Waived off for this job.

7.0 GUARANTEE:
Not applicable.

8.0 TERMINATION OF THE CONTRACT:

8.1 The Contractor should note that the Contract may be terminated at any period of time during the
Contract tenure with one week's notice in advance at the discretion of RIL without assigning any reason
thereof.

8.2 If contractor is unable to comply with the safety rules & regulations, RIL management has the right to
terminate the subject contract at any stage of work & get the job done with alternate Vendor at contractor's
risk & cost.

9.0 EXTENSION OF THE CONTRACT:


Contractor shall be committed to extend the validity of the Contract till the Contractual agreement tenure.

ANNEXURE III : Contractor Personnel's


Roles/Responsibilities/Accountabilities for Annual
Contracts and OTCs are to ensure following:
To ensure Safety of contractor's personnel and all other persons working around their work place.
To Conserve environment.
To Study and Comply with General Standards and Job specific Standards given in Work Order. (Examples
are standards for RA,TBRA. Scaffolding, working at heights, working in confined space, using proper
PPE's, proper tools, proper equipments, proper procedures.
To Observe safe practices & procedures communicated time and again.
To discharge responsibility for Contractor's own safety performance.
To Communicate clearly to your each person on RIL NMD's safety requirement
To monitor, improve and sustain Safety Performance of your company.
To Establish safety record for above given standard.
To make Improvement in overall safety performance of your company and make it visible and measurable.
To study & comply with basic safety rules and ensure your personnel follow it in RIL NMD.
To bring manpower at workplace only after Safety Induction Training is taken and each one have been
validated.
Validity of this certificate shall be 12 Mnths. You shall get it revalidated.
To engage a qualified Safety Representative as per job requirement.
To ensure Safety Representative shall train youir workmen, keep records of training and organize refresher
courses. programmes through contractor safety Management representative. as per the following heads.

Location of assembly points.

To conduct Toolbox talk. To ensure all tools /Tackles and accessories are complying with statutory
requirements. To conduct internal Safety audits by our safety representative for your resources.
To stop the work when found working unsafely, until the situation is rectified. Delay in work as a result of
such stoppages shall be regarded as a breach of contractual obligations. To avoid any serious violation by
the contractor,You shall keep re
o ensure that there is no violation of safety standards. In case of repeated non compliance/safety
violations, such contractor's name shall be removed from the approved contractors list.
Accidents/Incidents Reporting:
To report all accidents, incidents, property damage and near miss cases pertaining to contractor activities to
EIC.
To report to the plant personnel , in case of any injury to Contractor employee.If no body is available, you
Project superintendent/Site Incharge/Proprietor/ForemanisSafety resource(Safety Representative)/All
workmen
To ensure that all tools and equipment used by his working crews are maintained in good working order and
have been tested by the competent authority as per requirement of Factories Act and Maharashtra

To consult the EIC prior to changing supervisory personnel so that continuity of the contractor's supervision
is maintained in order to meet the required safety standards.
To nominate safety representative as a member of concerned plant safety committee and to ensure that
member so nominated takes active participation in regular meetings of these committees.
He is responsible for overall coordination of the safety procedures prescribed herein:
To work closely with concerned field engineer/shift supervisor and area safety officer/manger.
To facilitate RIL Personnel to carry out safety inspections of his site.
To comply with recommendations given by EIC.
To attend regular meetings of safety committee whenever called by EIC.
To get information from EIC on hazards associated with a particular job and to ensure that necessary
precautions are taken during the execution of that job accordingly.
To take action in consultation with EIC to rectify the unsafe conditions in work site prior to start of the work.
When work is in restricted area, to select personnel experienced with the work and will ensure that all safety
requirements are being adhered to.
To review the work area for unsafe practices and conditions and initiate corrective actions as required.
To maintain a constant awareness of fire hazards in their area of work and to take action to eliminate the
fire hazards if possible. Matters beyond his ability to handle shall be reported to EIC.
To know the plant emergency procedures to broaden the leadership base, which may be required to cope
up with serious accidents, personal injuries, fires or evacuation.
To provide all relevant information to incident investigation team for any incident resulting in personal injury,
environmental damage within his limit.
To deliver 5-10 minute safety talk every week to group of contractor personnel under his control. A group
should not be more than 15 people at a time.

8.2 SAFETY ITEM SPECIFICATION FOR CONTRACTORS.


Safety Helmet: Molded out of high impact, heat & chemical resistant HDPE with brim for additional side
protection, with 6 point ergonomic adjustable head band & chin strap. Approved by BIS specification no.
IS:2925-1984 approved by CMRS/DGMS/CLI . Helmet should be of yellow color.

Safety Goggles: Toughened/ Polycarbonate scratch resistant lense with side shield, Optically correct zero
power, impact resistant. Approved by BIS specification no. IS: 7524 Part-I

Safety Harness with double lifeline : Waist belt with shoulder strap (full body harness), 6mm thick coated
friction buckle& joint less D-ring, 44 mm wide nylon webbing, padded back, nylon stitched along with copper
rivets for additional safety, "quick fit" spring loaded hook. Approved by BIS specification no. IS:3521.

Safety Shoes: High ankle shoes, made from fine quality plain black leather, padded collar, full below
attached tongue, with steel toe cap as per IS 5852 with direct injection PVC Nitrile heel sole resistant to

SPECIFIC SAFETY REQUIREMENTS
AA: PPE requirements for contractors are:

1. Mandatory Basic PPEs in contractor's scope.


2. Job specific PPEs other than Basic PPEs in contractor's scope, and
3. Job specific PPEs other than Basic PPEs in RIL NMD's scope e.g. SCBA, canister masks.

EIC shall validate quality of PPE on Job, assess compliance and enforce it's compliance on job. In case of
noncompliance, EIC shall impose penalty as devised by CMG.

BB: Resource Requirement:-


Requirement of quantity of qualified, Trained, Experienced Technical personnel and Safety Officer,
Requirement of quantity & quality of tools & tackles, Quantity & Quality of equipments supplied by
standard manufacturers.

EIC shall validate quality & Quantity of resources on Job, shall impose penalty for noncompliance as
devised by CMG.

CC: Housekeeping Requirement :-

House keeping shall be required on every day basis and shall be completed before contractor leaves the
workplace every day.

DD: Training by HSEF :-

HSEF shall Impart training to new entrants , Refresher safety training for old staff and validate before
renewal date. The topics for training shall be Use of Mandatory PPEs, Special PPEs in contractopr's scope
and PPEs in RIL NMD's scope, Knowledge of occupational health for contract workmen on similar lines as
given for RIL -NMD employees, fire fighting training certification & drills for contractor and on many subjects
which are related with the safety of the contractor.

EE: General Safety Requirement :-

1. More safety roles and responsibilities clauses to contracts shall be included time and again to upgrade
safety requirements and all contractor supervisory and/or management personnel and their safety officers
shall be responsible and accountable for compliance.

2. Safety Training shall be imparted by HSEF/RIL Faculty to contractor's personnel and Training faculty
shall Conduct Examinations after every safety orientation session and safety training course and validate
trainee's performance. Those attendees who fail the exam shall repeat the safety orientation session or
training course and appear for the exam again. This will decide workman's eligibility for gate pass.

3. RIL Audit Team Members shall regularly walk-through and conduct field audits of contractor safety
performance

4. RIL Audit Team Members shall compile the results of the observations from field audits, analyze for
trends and (unsafe situations) corrected. Contractor supervisors shall be accountable for the corrections
and continuous improvement of safety.

5. EIC/Audit Team Member/Any RIL NMD Representative shall give recognition "On-the-spot" for
demonstrating excellent safety performance which should be a round the year activity. Contractor
themselves also shall use recognition methodology for their workers to motivate workers to prevent
incidents.

6: Contractor is informed that


a. RIL will audit all aspects of safety during the performance of the work to enforce compliance to the
applicable standards and procedures.
b. Contractors are not allowed to perform any work in an unsafe manner.
c. Consistent, unacceptable safety performance could result in termination of the contract.
d. Failure to implement their proposed safety action plan and consequent failure to demonstrate continuous
improvement of safety performance during the work could result in removal from the approved vendors' list.

7: Contractor is advised that

a. Workers engaging in work activities that cause imminent threat of injury shall be
stopped and resuming the work shall proceed after the supervisor and/or manager has
corrected all unsafe acts and unsafe conditions.
b. Repeated failure to correct unsafe acts and unsafe conditions could result in dismissal
of supervisory and/or management staff.
c. Continuous improvement of safety performance over the duration of the work is
expected and will be confirmed through frequent audits of the contractor's activities.
d. To perform regular audits on their own.

8: Small description on or definition of a near miss to the general safety clauses should be incorporated as
awareness and contractors should be vigilant to liquidate near miss conditions. Also reporting of near
misses and minor injury incidents by contractor workers to EIC should be emphasized strongly with reward
scheme to motivate near miss reporting.

9 : Incident Investigation of the contractor incidents shall be conducted by the contractor, whether alone or
jointly with RIL. Contractor supervisor shall be responsible to determine the root causes of the incident and
correct the causes expediently. Accountability for implementing recommendations for corrective or
preventive actions in a timely manner should be emphasized.

10: Contractors to organize and convene their own separate safety meetings without RIL in attendance (or
one RIL representative, maximum). Minutes of these contractor safety meetings should be sent to the RIL
"EIC" (line organization) for review and comment.

associated risk assessment documents like RA, TBRA etc also. The permit and risk assessment should
document the communication and agreement between RIL and the contractor.

12: Performance Evaluation during contract execution period:


a. Both the periodic and the "close out" contractor performance evaluations shall be done and contractor
shall upgrade it by implementing interactive feedback to and from both parties regarding safety.
b. Composite field audit data including averages and trends over the life of the work shall be informed to
contractor and contractor shall upgrade it by implementing the feedback to the contractor.
c. Evaluation for Qualitative comments regarding safety attitude, flexibility and cooperation shall be done
and contractor shall improve in those areas also.

13: At the "close out" evaluation, RIL shall inform the decision of whether or not the contractor will remain
on the approved vendors list , based on the performance of the contractor on Safety and Technical rating.

FF: Special Safety Requirement :-

1: Contractor shall inform if they are planning to engage the subcontractor. In that case, contractor to
confirm that responsibility and accountability of compliance with the Statutory regulations, Safety
Compliance and Technical competency of the sub contractor shall be responsibility of the Prime contractor.

2: Contractor to furnish information on


a previous successful performance at RIL or at other companies w.r.t. job.
b. resources like qualified, Trained, Experienced personnel and Quantity in
Number as required to complete the work safely.
c. quantity & quality of tools & tackles, Quantity & Quality of equipments
supplied by standard manufacturers to complete the work safely.
d. Methodology to adequately monitor subcontractor's activity.
e. Deploying Supervision by Safety Officer for safety aspects of the work,

g. System to conduct job safety inspection and maintain incident's record.
h. Confirmation that Contractor is insured and bounded.

audit records are with measurable results w.r.t certification and liasoning of their documentation of
safety training, defined safety roles and responsibilities for every one in the contractor's organization,
management, supervision, workers etc

3: Contractor to submit list of certificates and licenses for contractor employees who will perform the work.

GENERAL SAFETY RULES


1. Every contracting agency and their employees shall follow all the General Safety Rules strictly. In case of
any doubt, contractor should contact concerned RIL Officer or Safety Department for clarification. No one
shall willfully or due to negligence violate these safety rules.
2. No one shall enter the complex premises without valid gate pass and shall follow all security norms in the
complex premises.
3. Complex has been notified as "NO SMOKING ZONE", therefore smoking requisites are not allowed
inside the complex and they should be deposited at the entry gate with security.
4. Mobile phones are not allowed unless permitted by competent authority.
5. No spark or flame producing appliances or equipments are allowed inside the complex. All such material
should be deposited at entry gate with security.
6. Possession or consumption of intoxicating liquor or any drugs inside the complex is strictly prohibited and
if found, it will be seriously viewed.
7. Fighting or attempting to hurt others inside the complex premises is strictly prohibited and violation of
criminal law or committing an offence involving moral turpitude, shall be seriously viewed.
8. Carrying arms, ammunition and any other incendiary material inside the complex is strictly prohibited and
if found with such material, shall be seriously viewed.
9. Sleeping in company premises is strictly prohibited.
10. Horseplay or loitering inside complex premises is strictly forbidden and therefore all the contractor's
employees should remain only at their work place and work seriously as per instructions.
11. Every employee of the contractor as well as the proprietor using his own vehicle, should observe "
Speed Limit " (40 KM per hour maximum) inside the complex and those using two wheeler shall wear "
Crash Helmet" including pillion rider. Also four wheeler driver & co passenger shall wear safety seat belt.
12. No vehicle shall enter the complex premises without approved Spark arrestor/Muffler, fixed at the
exhaust pipe of vehicle.
13. Malicious act resulting in injury to any employee or damage to property is strictly prohibited.
14. Every contractor and their employees shall follow regulations of entry into the Battery Limit of the Plant
strictly and contractor supervisor shall always report to Control Room to seek the permission to enter the
plant.
15. Entry of any vehicle inside the Battery Limit of the plant is prohibited unless otherwise authorized under
permit from Control Room.
16. No contractor or their employees shall carry out any job either hot or cold inside Plant Battery Limit as
well as in non-plant areas unless the permit to work is obtained from the Control Room/ concerned area-in
charge.
17. No contractor or his employee shall tamper with or mishandle any of the equipment of the RIL.
18. No contractor or his employee shall steal or make fraudulent records or knowingly obtain material on
fraudulent order.
19. In case of any emergency in the plant, contractors shall suspend all their activities and come out with all
his employees and assemble at "Assembly Point " till further instructions.
20. In case of any accident, injury or fire, it should be immediately reported to Control Room, or concerned
officer of RIL.
21. Fire water should not be used for any job unless it is permitted.
22. All contractors' employees shall wear "Safety Helmet, Safety Shoes, Safety Goggle and Hand Gloves,
EAR PLUG" on regular basis as and when they are inside the plant ISBL area and shall make use of all
other Personal Protective Equipments (PPEs) as per requirements specified in work permit.
23. It will be the responsibility of the contractor to see that the special safety equipment / appliances
supplied by the RIL-NMD are used with utmost care and if there is any damage to such equipments then
cost shall be recovered from the contractor.
24. Every contractor shall use right / suitable tools for the job to ensure safety.

25. All workmen of the contractor shall be properly dressed (work uniform with company name) and nobody
shall wear loose clothing, which may cause injury/damage while working with rotating machinery.
26. Contractor shall ensure that authorised & trained electrician shall carry out all electrical work.
27. Negligence or carelessness of any contractor or his employee(s) against the interest of RIL-NMD shall
be viewed seriously.
28. No contractor or his employee shall refuse to perform assigned work and disregard intentionally or
disobey the instructions.
29. It will be the prime responsibility of the supervisor or the Engineer-in-charge of the contractor to see and
ensure that all the Safety Rules are strictly complied and followed without any excuse whatsoever and shall
demonstrate competency and ability to do the work, which had been agreed upon while awarding the job.

In addition to all above General Safety Rules, each contractor shall ensure that all his workmen undergo
induction and regular safety training as and when it is arranged by Plant / Dept. Engineer I/C / Safety
Department.

CONTRACT

Applicability :
i.Contractor deploying No. of workmen more than 20 at site shall deploy
independent Contractor Safety Representative.
ii.Contractor deploying workmen less than 20 at site shall themselves function as
Contractor Safety Representative.

Functions are as follows :

1. To ensure that Qualified, Trained & Experienced Technical Personnel are mobilised and are fully
conversant with Safety requirements of the job.
2. To ensure Safety and Statutory requirements are complied for tools & tackles used by the contractor's
workmen. To ensure that they are in good condition, safe to use, certified for use, certified by RIL-NMD
Engineer in case of Electrical machinery.
3. To ensure that Mandatory PPEs & Job Specific PPEs are used by Contractor's workmen whenever in the
field.
4. To ensure that General and Job specific Roles, Responsibilities and accountabilities of contractor's
Workers, Supervisor, Safety Representative are complied.
5. To ensure that Road Safety Management System is complied.
6. To ensure that all Safety requirements are complied by the Sub Contractors workmen.
7. To ensure that Safety Programme/Safety action plan for the job execution finalised in the pre-bid opening
meeting is reviewed and followed.
8. To ensure that safety inspection is carried out continuously to avoid any injury and incase of any injury,
record is maintained.
9. To study General, Special and/or unique safety requirements/ procedures applicable to Job thoroughly
and ensure that environmental harm, property damage, and business losses by contracted activities are
totally prevented .
10. To ensure that Safety roles and responsibilities clauses amended during the contractual period are
followed by all your employees.
11. To ensure that House keeping is completed before workmen leave the workplace every day.

13. Training and orientation:
a. Contractor Safety Representative shall co-ordinate with Contract Administrator/Learning Centre for the
Orientation & safety training before mobilization. He shall ensure that all workmen are trained before
mobilizing in the plant.
b. The training areas are as follows:-
I. General Safety Training.
II Plant Related Safety Training and
III Job Specific Safety Training.
IV Rules and regulation to be followed at site.
c. Learning Centre shall validate workman's performance for each Safety Orientation session. Trainees
failed shall repeat the Programme till workman is validated.
d. Contractor's Safety Representative shall keep record of orientation and safety training attendance and
examination marks.
e. Contractor's Safety Representative shall ensure that untrained workmen is not deployed on job.
14. Contractor Safety Representative shall have his own safety management system for safe functioning at
site.

He shall:

a. Organize Monthly safety meeting with their staff with or without RIL representative, prepare and send
MOM to EIC for review. EIC shall advice for improvements and shall liquidate EIC's recommendations.
b. Conduct Self safety audits every day and keep record on improvements.
c. Conduct Tool Box Talk every day , cover various topics and keep record of subjects covered.
d. Train contractor's workmen on Electrical equipment area classification and shall ensure that all
equipments are provided as per electrical area classification.
e. Ensure contractor's workmen do not carry items like mobile phone , match box, cigarette or any ignition
source in battery limit area. In case found carrying, his gate pass shall be cancelled permanently and
Contractor Safety Representative shall be penalised.
f. Ensure that truck, tanker, fork lift drivers are trained and always carry driving license.
g. Train contractor workmen on Safety precautions required as per MSDS for the materials handled by
workman.
h. Ensure that Work permits associated with risk assessment documents like RA, TBRA etc are discussed
with EIC are understood fully before starting the job and sign the document.
i. Ensure that near misses and minor injury incidents by contractor workmen are reported to EIC and
recommendation of EIC are liquidated within time given by EIC. Concept of near miss shall be explained in
the Training Programme.
j. Ensure that Contractor Safety Representative may be involved in conducting Incident Investigation of the
contractor's incidents, with RIL to determine the root cause of the incident. Contractor's Safety
Representative shall liquidate recommendations within time given by EIC.Concept of Incident Investigation
shall be explained in the Training Programme.
k. Give accurate and quick head count of contractor workers when involved in Safety drills conducted by
plant HOD..

14. Contractor Safety Representative to note that


a. work shall be stopped in case contract employees are engaged in work having threat of injury. Work shall
be started only after the contractor has corrected all unsafe acts and unsafe conditions.
b. Contractor workman shall be dismissed in case fails to correct unsafe acts and unsafe conditions are
repeated.
c. Contractor Safety Representative's efforts for Continuous improvement of safety performance shall be
evaluated.EIC shall take action if improvements are not seen.

to the work for safety of our employees and property of the company (RIL) and provide required personal
protective equipment to all our employees, as necessary for safety & to prevent probable injuries to our
workers and as per the safety requirements of the Company (RIL). We further undertake to carry out the
work in accordance with the safety policy and procedures of RIL

6. That we shall also comply with all applicable regulations with respect to environmental protection.

7. That we undertake to deploy competent and trained personnel for the job identified who will possess
requisite skill, certificate/ license.

8. That we shall ensure use of right type of tools, tackles & equipment, maintained in good working
condition, suitable to the nature of the job under execution.

9. That we shall be responsible for safety of our employees and other persons at site/ property of the
company and accountable for any accident due to negligence/ fault of our employees or our company and
shall be ready to accept the responsibilities for any legal consequences.

10. If we are unable to comply with the safety rules & regulations, RIL management has the right to
terminate the subject contract at any stage of work done & get the job done with alternate Vendor at our risk

the contractor (hereinafter referred as the 'Laws').
AND WHEREAS the Contractor has agreed to indemnify and keep the
Company and the management indemnified against all claims,
charges, penalty, fine or any other levy against the Company
and/or the management due to the failure of the Contractor to
observe or follow any legal and statutory provision and the
Contractor has agreed to execute an indemnity bond to this effect.
NOW THIS DEED WITNESSETH AS UNDER:
1. In consideration of the Company awarding a contract to the
Contractor subject to the condition of execution of this
declaration and indemnity bond the Contractor declares and
represents to the Company that the Contractor has obtained all
statutory registrations, certificates, licenses and approvals
required under the law enabling the Contractor to execute the
contract in a legal and lawful manner.
2. THAT in the event of any liability arising out of failure to
observe or non-compliance of any such 'Laws' by the Contractor in
discharge of the said contract, the Contractor shall bear all the
resultant whatsoever liability(ies), if any arising out thereof
and that the Company and the management shall not be liable for
any such liability(ies). The Contractor agrees to indemnify and
keep the Company and the management indemnified against all
losses, costs, damages, claims, penalties, interest, expenses,
demands, fines, legal liability, causes of action, injury to

persons, etc which may be suffered, incurred, undergone and / or


sustained by the Company including the costs and expenses that may
be incurred in defending any such liability(ies) claim(s),
proceeding(s) etc. that may be made or taken or arise on the same
by any person, body, authority, government, judicial / quasi
judicial authority due to the failure or non compliance of any
such laws and rules there under (including any amendments in acts,
laws, statutes & rules there under) of whatsoever kind and nature
arising out of or in any way connected with, whether or not such
acts or omissions are actual or alleged, active or passive with
regard to the discharge of the said contract.
3. This declaration cum indemnity bond shall be in accordance with
the laws of India and any dispute between the parties as regards
the contract of indemnity shall be settled in accordance with the
provisions of Indian Arbitration and Conciliation Act, 1996,
including the method and manner of appointment of the Arbitrators.
