Abstract
Patients with Chronic Kidney Disease (CKD) who enter the terminal
stage require renal replacement therapy and self-care for the rest of their lives.
PGK patients with CAPD use should be prepared for the treatment of renal
replacement therapy and self-care to be more optimal in managing the disease, one
of them by providing empowerment based education (self-empowerment). Nurses
play a significant role in facilitating education that empowers patients to improve
self-efficacy of patients in self-care management. Senior Practical Writing aims to
apply the science of empowerment-based education in patients with chronic renal
failure in RSUP H. Adam Malik with assessment process, nursing diagnosis,
intervention, implementation, and evaluation. Number of patients under 1 person,
determined by medical diagnosis. The authors conducted an assessment of the
patient for one day, determined the nursing problems experienced by the patient is
the pattern of ineffective breathing, tissue perfusion disorders, excess fluid volume,
then the authors set nursing interventions and evaluate patient development lasting
for 3 (three) days. The patient's health condition improves with intervention.
Further recommended, the nurse makes special handling procedures for nursing
problems experienced by patients with chronic renal failure as self-care nursing
intervention. One of the application of empowerment based education in patients
with chronic renal failure, because empowerment-based education is Evidence
Based Practice (EBV).
