AFFIDAVIT

I, [NAME] Filipino, of legal age, with present address at __________________________, hereby

depose and say:

1. That I intended to mail a parcel to the Philippines through the Philippine Postal
Corporation (“PHLPOST”) to [NAME], Filipino, of legal age, with permanent address
at [ADDRESS];

2. That, through inadvertence, the third person whom I instructed to mail the parcel to
[NAME] mistakenly wrote down [WRONG NAME] instead of [NAME] in the mailing
form;

3. That I immediately informed [NAME] of the inadvertent error through Facebook

Messenger (attached herewith as Annex “A”);

4. That [NAME] received PHLPOST Form [NO.] for “REGISTRY/PARCEL [NO.]” on 24

February 2017, with the name thereon indicated being the incorrect [WRONG NAME]
(attached herewith as Annex “B”);

5. That [NAME] is the true intended recipient of “REGISTRY/PARCEL SP 1886 ATP” and
not the erroneous [WRONG NAME];

6. Through this AFFIDAVIT, I hereby instruct and authorize [NAME] to present my

_________ ID (attached herewith as Annex “C”) and claim the parcel marked
“REGISTRY/PARCEL [NO.]”;

7. I am executing this AFFIDAVIT to attest the truth of all the foregoing, and for all legal
purposes that it may serve.
