AUTHORIZATION LETTER

To whom it may concern,

This is to authorize, NAME HERE, to receive the PSA certificate of my daughter on my

behalf. I’m not available to receive the document as my schedule doesn’t permit me to be
present hence, assigning an authorized representative.

I have also attached my valid ID, together with the valid ID of the aforementioned
authorized representative.
