Subject: Authorization to Claim my Diploma and its Certified True Copy

Greetings of Peace!

I, Fatima Saida A. Maing, hereby authorized Mr. Kimhar J. Dambong to act as my

representative and claim my Diploma and its Certified True Copy which I requested
from your good office on June 15, 2023. I cannot come in person to claim the said
documents within the date prescribed because I am currently in Tawi-Tawi and doing
so in person would entail me considerable amount for transportation expense, hence,
this authorization is made.

Furthermore, I hereby authorized Mr. Dambong sign and submit necessary document/s
in relation to the process of releasing and claim of my Diploma and its Certified True
Copy.

Attached in this letter are copy of my claim stub and Identification Card for your
reference.
