BRENFRED NALAZON ROMERO

Admissions Committee
University of Eastern Philippines Main Campus
Catarman, Northern Samar

Dear Mr. Romero;

I hope this letter finds you in good health and high spirits. I am writing to address a concern
regarding the submission of my live birth certificate as part of the requirements for my college
admission to the University of Eastern Philippines.

Due to unforeseen circumstances, I currently do not possess a copy, either original or photocopy,
of my live birth certificate. The majority of my documents are presently located in Manila, and I
am in the process of having them shipped to my current location. However, it is unlikely that
these documents will arrive before the deadline for submission, which is 15th of July,2023.

I would like to assure the admission committee that I am fully committed to complying with the
necessary requirements and providing all the supporting documents at the earliest possible time. I
understand the significance of the live birth certificate and its importance in validating my
identity and eligibility for admission.

Therefore, I hereby request an extension of the deadline for the submission of my live birth
certificate. I anticipate that the shipment containing my documents will arrive on or before 1st or
2nd week of August, 2023. Upon receipt, I will promptly submit the required live birth certificate
to the admission office. I apologize for any inconvenience caused by this delay and assure you
that I will prioritize the submission of the live birth certificate as soon as it becomes available to
me.

Thank you for your understanding and consideration in this matter. Should you require any
additional information or have any questions, please do not hesitate to contact me at the provided
email address or contact number.

Yours sincerely,

Julliene Radam Espiña
