This letter seeks to EXPLAIN and at the same time extend my genuine apology for being absent from

duty from 28 April- 30 April 2023.

Last _______, the undersigned received an instruction regarding the submission of requirements to
update the beneficiaries under ___________ (ano ung sa beneficiaries, AFPSLAI BA?) including PSA Birth
Certificates, Certificate of No Marriage and ID pictures. Documents shall be submitted on or before 4
May 2023.

However, considering that I do not have in my possession the said requirements and that personal
appearance to the Philippine Statistics Authority (PSA Lipa) is required for a CENOMAR to be issued, I
have no other choice but to schedule and proceed with my appointment to obtain such document on 28
April 2023.

I asked my mother to meet me in PSA Lipa for her PSA Birth Certificate and for us to have an ID Photo
but much to our dismay, an emergency happened in our household and she cannot leave the house, so
at 5:45 PM of 28 April I decided to go home in San Juan, Batangas instead of returning to duty without
my documents.

It was only on 30 April that I and my mother was able to get an ID photo taking into account the
emergency situation in our household. I was able to return to my duty at around 8PM of 30 April after
an almost four hours of travel.

I do understand the verbal instruction from our FTO, and I do not negate the fact that I was absent from
duty. However, I was torn between such verbal instruction and the previous order regarding the
submission of documents for my beneficiary. With that, I am extending my genuine apology and I am
hoping that my reason be taken with merit and that this case be look at with compassionate grounds.
