Re: Authorization to Secure Death Certificate

Gentlemen/Mesdames:

late relative, Azucena O. Entica, who passed away on January 09, 2022. Due to personal
constraints, I am unable to visit the PSA office in person.

Gorgonio to facilitate this process. I kindly request your assistance in providing a certified
copy of the death certificate to Lilibeth Gorgonio.
