RE: Authorizing someone to claim my X-rat result

Greetings of joy!

I am writing this letter to inform your good office that I will not be able to claim my X-ray result due to
an unexpected errand I am tasked to do at work.

With regards to this, I am authorizing my sister, Jhea May G. Rabasano, of legal age, to claim my x-ray
examination result on my behalf since I am not around in the city center this time. It is very important
for me to claim the result since the company I am working at obliged us to submit an updated X-ray due
on Monday, January 16, 2023. I will back at work next week, so I do not have enough time to drop in
your center and claim the said document.

See attached receipt as your reference on day I was scheduled for the examination, as well as my
Identification cards. I am hoping for your consideration.
