Dear ____________,

Greetings! I am writing this letter to ask for your kind consideration with regard to the late
submission of the quarterly breeding reports for the Third and Fourth Quarter of the year 2019,
and that for the entire year of 2021 respectively. I am humbly asking your office that the
aforementioned reports be given due consideration despite their late submission so that me
and my farm can secure a Bureau of Animal Industry Animal Welfare Permit.

While I do understand that the responsibility to submit the said reports in a timely manner, falls
solely on me, certain unfavorable circumstances which were out of my control rendered the
timely submission of such reports extremely difficult. During the later parts of the year 2019,
my daughter, Patricia Nicolas, whom I entrusted to come up with the said Quarterly Breeding
Reports, fell ill due to Anemia and its complications. Along with that, she was also extremely
busy with her veterinary medicine studies especially during the later parts of the year. With her
extremely busy schedule and failing health at that time, she was not able to create the said
reports. Hence it caused us to not be able to submit the same on time that was prescribed.

Furthermore, during the year 2020, as we all know, the country faced the pandemic caused by
COVID 19. As such circumstances were new to all of us, there were so many uncertainties
especially with the rapid spread of the virus across the country. Because of this, we elected to
stay indoors as much as possible unless it was absolutely necessary. Unfortunately, this also led
to our late submission of the quarterly breeding report for the said year. We felt like doing so
would entail unnecessary risks which could have exposed me and my family to the perils of
being contaminated by the virus. I am also living with my parents and their relatives whom are
all senior citizens. Hence, it was a risk I simply could not afford to take at that time. More so, I
was also uncertain if your office would’ve been able to accommodate us at that time since we
were practically under strict quarantine protocols for the entire year.

While I do admit my failure to submit the said documents on time, I would also like to ask for
your very kind consideration for this instance. I was simply prioritizing the safety, health and
well-being of my family. Moving forward, I will make it a point to submit such documents in the
manner prescribed by your agency. Kindly accept our sincerest apologies and I am hoping for
your favorable and kind considerations for this matter.
