AFFIDAVIT IN SUPPORT OF LABOR CASE
I, NATSU DRAGNEEL, of legal age, Filipino, single, and a resident of Quezon City, after having
been duly sworn to in accordance with law, do hereby depose and state:
1. That I was employed by Fairy Tail, Inc. as a Fire Wielder in 2019 until 2023 when I was
dismissed from service.
2. Upon my start of work, I was issued a Company Identification Document showing my name,
photo, signature, date hired, my Social Security Contribution Number, my PhilHealth Number, the logo of
the company and the name and signature of Mr. Fu who is the general manager of the company.
3. As a Fire Wielder, my duties include serving the Fairy Tail guild through completion of different
tasks.
4. My first direct supervisor was Sir Gloyd, then my last direct supervisor was Sir Fu who sees to
it that I am doing my job well.
5. I was required to report to work between 7:30 o’clock in the morning up to 5:30 o’clock in the
afternoon with one hour lunch break every Monday to Friday, and every Saturday and Sunday on an on-
call basis, with overtime work when necessary. My attendance was being monitored using the company’s
magic time card.
6. I received my wages twice a month, that was, every 15th day and last day of the month. I have
attached herewith a copy of my available pay slip.
7. Being the breadwinner of my family whereby my said job was my only source of income and
considering that my salary was only four thousand pesos (P4,000.00) daily, I have been very diligent in my
work and have not incurred any absences since the start of my job in 2019 until its termination.
8. In 2023, I was devastated when the company’s Human Resource Officer verbally told me that it
was my last day on the job.
9. I did not receive any prior written notice nor any formal charge, there is even no opportunity for
me to be heard. My dismissal from service was made arbitrarily as it was only done verbally and I believe
that it was not based on any just cause, and even if it is based on an authorized cause, I was not notified
properly. I was not even made to sign any papers/documents. Such that, I have been deprived of substantive
and procedural due process.
10. That I am executing this Affidavit to attest to the truth of the foregoing statements, to support
my case for illegal dismissal, and for whatever legal purpose(s) this may serve.
