Dear Sir,

Good day! We are third year BS Psychology students in Our Lady of Fatima University. In partial
requirement of our Research B, we will need to conduct a study entitle “A Rhizomatic story approach on
the experiences of Filipino family caregivers of end stage renal disease dialysis patients”. Our study aims
to get the narratives of the caregivers specifically on their experiences handling patients with End Stage
Renal Disease (ESRD) to promote awareness and social support to lessen the “burden” particularly on
caregiver’s mental health.

In line with this, we are requesting for your permission to allow us to conduct this study outside the
campus. We will be interviewing 4 family caregivers of patients with end stage renal disease. The
participants and the researchers will agree upon a time and location for the interviews. Ethical standards
and guidelines will be properly observed.
