Materials:
1. High School registration packets
2. Pencil and paper
3. Pre-post survey
Lesson Outline/Procedure: Include each of your learning activities
1. Introduction
2. Hand out survey, ask student to write what they know about on time graduation requirements.
3. Present information and high school registration packets.
4. Activity: Have student create a paper booklet to record graduation credit requirements.
5. Go through all pages of registration packet.
Plan for Evaluation: How will each of the following be collected?
Process Data:​ Attendance
Perception Data:​ Students complete a survey at the beginning of the lesson defining what they know about on time
graduation requirements. Students complete a post survey answering the same question at the end of the lesson.
Outcome Data:​ Number of completed high school registration packets ready on registration day.
Plan for Next Lesson/ Follow Up:​ Review of data perception data with school administration
