AFFIDAVIT OF DESISTANCE

I, “COMPLAINANT”, of legal age, Filipino, with postal address at

San Isidro Magalang, after having been sworn in accordance with law, do
hereby depose and state that:

1. I am the private complainant in the CRIMINAL CASE NO. R-

ANG-15-00709-CR against “ACCUSED”, pending before the Regional
Trial Court of Angeles City, Branch 60.

2. After calm reflection, and deeper evaluation of the facts of this

case I decide not to pursue the case against him as I have forgiven him
already..

3. In view of the foregoing, I am no longer interested in further

prosecuting the abovementioned case against “ACCUSED”.

4. I am executing this affidavit to attest to the truth of the

foregoing and to have the present case dismissed with prejudice.

IN WITNESS WHEREOF, I have hereunto set my hand this 29th day

of June 2016, in the City of Angeles Pampanga.
