Japan Visa Requirements

1. Philippine Passport (if new passport page 3 must be signed.)

2. Visa Application Form
3. Photo (Specs: 4.5cm x 4.5cm, with white background.)Japan Visa Size
a. Photo must be taken within 6 months.
4. Birth Certificate (issued within one year from PSA)
a. If the birth certificate is “late registration”, please submit a baptismal certificate and school record (Form 137
b. If there is no record in PSA, please submit a “Certificate of Non-Record” from PSA together with a birth
certificate from the Local Civil Registrar.
5. Marriage Certificate (if the applicant is married) issued within one year from PSA
 if self employed/working
6. Bank Certificate
7. Applicant’s Income Tax Return (Form 2316) clear Photocopy (latest)
8. Certificate if Employment together with company ID( if employed) Certificate of Business Registration (if Self-employed)
 if Student/ minor
1. School Registration form or certificate of enrollment together with School ID (if student)
2. Guarantee Letter from parents
