1. Application Form (A4 size paper)

2. 2 pieces of Passport size colored picture

4. Photocopy of Passport Bio-Page on page 2 (A4 Size paper)
5. Original & Photocopy of Valid visa/s and arrival stamps to OECD member countries for the past 5 years (if
applicable only)
6. Original PSA Marriage Certificate
7. Additional Documents
 If husband is employed:
 Husband’s employment Certificate original must include position, date hired, compensation,
office address, HR landline number [cellphone number is not allowed, HR e-mail address].
 Personal Bank Certificate must include account type, account number, currency, current balance,
6 months average daily balance, opening date, issues within a month.
 Bank Statement original or certified true copy of bank statements/passbook for the last 6
months, issued within a month.
 Photocopy of ITR (Income Tax Return) or Form 2316 copy, ITR submitted should include the
pages with net income and total revenue.
 Letter of Attorney (signed per application) notarization is not required, signature of parent or
legal Guardian for 17 years old and below.
 For family applications, documents as proof of relationship is a must. ORIGINAL PSA COPY
(Marriage Certificate/Birth Certificate, etc.) Family application should be filed together at the
same time
 Notarized Affidavit of Support (if applying with guarantor/sponsor)
 Scanned Passport or any valid government Issued ID.
 PRC Card or IBP Card (if applicable)
 If husband is self-employed:
 Business Registration from SEC or DTI (Copy) Business permit or Mayor’s Permit (Copy).
 Personal Bank Certificate must include account type, account number, currency, current balance,
6 months average daily balance, opening date, issues within a month.

months, issued within a month.
 Photocopy of ITR (Income Tax Return) or Form 2316 copy, ITR submitted should include the
