SOUTH KOREA VISA REQUIREMENT

GENERAL REQUIREMENTS

1. Duly accomplished and signed application form (must be computerized, font style
and font size is Arial 12 only; must be printed in A4 sized paper))
2. 1 pc passport sized colored picture (white background)
3. Original passport (at least 6 months valid beyond intended stay)
4. Copy of passport data page
5. Original copy of valid visas and arrival stamps to OECD member countries for the
past 5 years
6. Original personal bank certificate (must include account type, current balance,
account opening date, and average daily balance for the last 6months).
7. Original bank statement or certified true copy of passbook for the last 3 months
8. Photocopy of ITR (Income Tax Return) or Form 2316
9. Original PSA Marriage contract (if applicable)
10. Original PSA Birth certificate (if applicable)

FOR RETIREE

1. Copy of senior citizen card or retirement documents

2. Original of PSA marriage contract (If travelling with spouse)

IF STUDENT TRAVELLING WITH PARENTS

1. Original school certification

2. Copy of school ID
3. Original PSA birth certificate
4. Parent's documents (original bank certificate and bank statement or passbook for
the last 3 months, original COE, copy business documents and ITR)
5. Original PSA marriage certificate

IF SOMEONE WILL SPONSOR

1. Affidavit of Support and Guarantee/Letter of Guarantee

2. Financial Documents of the sponsor
