We are enclosing herewith form 'B','B1', Certificate and an amount of


Rs. towards the licence fee has deposited in the Government Treasury and
request you to grant a licence to use the lift at an early date.

The installation is completed by and we would appreciate if the inspection is carried out
immediately to enable us to use the lift.

Please let us know when you intend to visit the site to inspect the installation so that we
could ask M/sto make arrangements to depute one of their representatives to assist you
in carrying out the inspection.

Also enclosed find a Certificate & Form 'B1` required by you in accordance with the
requirements of for the lift at the above premises.
