PETITION

COMES NOW the undersigned attorney in behalf of the petitioner, and unto this
Honorable Court, respectfully avers:

1. That the petitioner who is of age, single, and resident of

2. That the said minor ____________________, who at the time of the filing of
this petition is only __________ years of age, is a resident of

3. That the said minor is seized of real and personal property, located in

convenient that a guardian of his person and property be appointed
