AFFIDAVIT
I, JENELYN E. VILLALUNA, of legal age, Filipino, single, and a resident of Brgy.
Handumanan, Bacolod City, Negros Occidental, Philippines after having been sworn to in
accordance with law, hereby depose and state that:

1. I am an incoming 3rd Year BS Accounting Technology Student at La Consolacion College

(LCC) – Bacolod;

2. Sometime in the Second Semester of School Year 2018-2019, I was cited by the Discipline
and Student Affairs (DSA) for violation of school policy of which a receive sanction;

3. Said violation involved sporting a dyed hair during the school year;

4. I was not able to serve the sanction for the said violation last school year due to heavy
school load and academic requirements;

5. I hereby undertake and voluntarily impose upon myself the obligation to abide by the
school policy of LCC-Bacolod, particularly on wearing my natural hair color;

6. I further undertake to have my hair dyed back to its natural color (black) by the 2nd Week
of July under pain of school sanction should I fail to make good of my undertaking on the
time given;

7. I am executing this affidavit to attest to the veracity of my statements and in support of

my intention to enroll in the First Semester of School Year 2019-2020 at LCC-Bacolod;
