LETTER OF CONSENT

TO WHOM IT MAY CONCERN:

_____________________________(Student), DO HEREBY GIVE OUR CONSENT
for him/her to attend a youth leadership training and award conference sponsored by the
Rotary Clubs of The Bahamas to be held in Nassau, Bahamas during the weekend of Feb
24 – 26, 2017. As a result of my child participating in this conference he/she will not
attend school on Friday the 24, 2017.

This Letter of Consent is being issued this ____ day of _______________ 2017 at
