AFFIDAVIT
I, ELRIC ESTA VILLENO, Filipino, of legal age, and a resident of Zone 6,
Brgy. Salvacion, Ocampo, Camarines Sur, do hereby, under oath, states that:

1. I am an active member of the Philippine National Police (PNP) with the

rank of Patrolman and currently stationed and assigned at the 502nd
Maneuver Company Patrol Base at Sitio Mararag, Brgy. San Jose, Libon,
Albay;

2. As an active member of the PNP, I was issued a service firearm by the

latter with serial number 92704 (Brand: Taurus TS9);

3. At about 1:00 o'clock in the afternoon of 9 June 2019, while I was

unloading my service firearm as is routine for every police officer to do
so every after end of duty, the said gun suddenly fired and accidentally
shot my left leg just above the knee causing the same to be severely
injured and which eventually got me rushed to the NICC Doctor's
Hospital in Naga City for immediate medical attention;

4. I hereby state under oath, that what happened was mere accident and
not intended by me nor by anyone;

5. This affidavit is being executed to attest to the truth of the foregoing

and for any legal purpose it may serve.
