Greetings!

The bearer of this letter, Mr. / Ms. __________________________________ is enrolled this semester in the
course, D-REED122 – Christian Discipleship in the Modern World. One of the objectives of this course is to
lead the students to a greater appreciation of the Church or community life by exposing and involving themselves
in the ministries/programs of their respective Christian/social/civic communities. Hence, they are required to
render at least ten (10) hours of service. This service should be undertaken for a period of at least five (5)
weeks.

In this regard, we would like to request that the bearer of this letter be given an opportunity to be exposed and be
involved in the activities of your Church/community by providing him/her tasks that will enable him/her understand
better his/her role and responsibility in the community. You can designate any personnel in one of your
ministries/programs to oversee the work of the student concerned.

Moreover, please be informed that upon the completion of at least 10-hour Church/community service, the
students will ask for a certification and assessment of his/her work. For the purpose of assessment, you will be
provided the necessary form to be filled up.
