Please complete this service-hour recording sheet in order to receive credit for
completing service hours. For the Bothwell chapter of the NJHS,
● There are four total hours of service required to participate in the recognition and
induction ceremony that takes place in April. Service hours are to be completed
by March 21, 2020.
● There is broad latitude in the type of service that can be counted towards fulfilling
your service obligation. Please contact Mr. Lambert to determine whether or not
your service to the community can be counted toward the four-hour service
requirement before you complete your hours.
● Submit this form to Mr. Lambert within 10 school days of your date of service.
Hours may not be granted if not submitted promptly.
● Service hours cannot be counted towards two different obligations. For example,
if you (or your child) play[s] for a hockey team that requires completion of service
hours, you may not count those service hours towards the NJHS.
